采用三维适形放疗联合紫杉醇和顺铂治疗食管癌放疗后复发患者的临床疗效

摘　　要：目的探讨采用三维适形放疗联合紫杉醇+顺铂(TP)方案治疗食管癌放疗后复发患者的临床疗效。方法选取2011年6月至2013年6月间北华大学附属医院收治的98例食管癌放疗后复发患者,根据治疗方式的不同分为观察组和对照组,每组各49例。观察组患者行三维适形放疗联合TP方案治疗,对照组患者行单纯三维适形放疗治疗,比较两组患者的临床疗效。结果观察组患者的治疗总有效率明显高于对照组(P<0.05),观察组患者的2年生存率明显高于对照组(P<0.05),两组患者的不良反应发生情况比较,差异有统计学意义(P<0.05)。结论采用三维适形放疗配合TP化疗治疗食管癌放疗后复发患者,可提高患者近期疗效及生存率,对于无化疗禁忌证的患者可考虑采用同步化疗。Objective To study the efficacy of three-dimensional conformal radiotherapy combined with paclitaxel ＋ cisplatin（ TP） regime in the treatment with esophageal cancer radiotherapy. Methods From June 2011 to June 2013 in Affiliated Hospital of North China University,98 cases with esophageal cancer recurrence after radiotherapy were enrolled in the study and were divided into the observation group and the control group,49 cases in each group. The observation group accepted three dimensional conformal radiotherapy in combination with TP chemotherapy and the control group only accepted simple three-dimensional conformal radiation therapy. The clinical efficacy of two groups were compared. Results The total effective rate of the observation group was obviously higher than that of the control group（ P〈0. 05）; The 2yr-survival rate of the observation group was obviously higher than that of the control group（ P〈0. 05）. The occurrence of adverse reaction of the two groups also showed significant differences（ P〈0. 05）. Conclusions For patients with esophageal cancer recurrence after radiotherapy,three-dimensional conformal radiotherapy and TP chemotherapy can improve the near future curative effect,and improve the patients＇ survival rate. Therefore,for the patients with no chemotherapy contraindications,synchronous chemotherapy may be considered.
