摘 要:目的 通过离体肝静脉及门静脉CT直接造影,研究肝静脉的解剖结构,为临床提供精确的影像学资料。方法离体肝脏解冻、负压吸引后,于肝静脉内充入气体、门静脉内注入碘对比剂,行CT扫描并进行三维重建,观察肝静脉汇入下腔静脉类型、肝静脉分型,并分别统计各肝静脉主要属支数目。结果 肝静脉汇入下腔静脉类型为:Ⅰ型73.7%(14/19),Ⅱ型21.1%(4/19)。肝右静脉分型为:A型占47.4%(9/1 9),B型占36.8%(7/19),C型占15.8%(3/19)。肝中静脉分型为:A型占52.6%(10/19),B型占26.3%(5/19),C型占21.1%(4/19)。肝左静脉分型为:Ⅰ型42.1%(8/19),Ⅱ型57.9%(11/19)。肝右、中、左静脉主要属支数目分别为4~10支、4~10支及3~7支。结论 通过离体肝静脉及门静脉CT直接造影,可以准确地显示肝静脉汇入下腔静脉类型、肝静脉解剖分型以及肝静脉在肝段的分布。Objective Through the direct CT angiography of hepatic vein and hepatic portal vein in vitro, to study ana- tomic structure of hepatic veins so as to provide accurate imaging data for clinical practice. Methods In vitro livers under- went thaw, vacuum aspiration. Then, enough gas was poured into hepatic vein and iodine contrast media was poured into portal vein. Subsequently, f CT scanning and 3D reconstruction were conducted to observe the inferior vena cava type as afflux into the hepatic veins, hepatic vein type, and for the statistics of the number of the main hepatic vein respectively. Results The types of hepatic veins into the inferior vena cava were as follows: type I , accounting for73.7%(14/19), type H , accounting for 21. 1%(4/19). The types of right hepatic vein were: type A, accounting for 47.4%(9/19), type B, accounting for 36.8%(7/19), type C, accounting for 15.8%(3/19). The types of middle hepatic vein were: type A, accounting for 5Z. 6 % ( 10 / 19 ), type B, accounting for 2 6.3 % ( 5 / 19), type C, accounting for 21.1 % ( 4 / 19 ). The types of left hepatic vein were: type I, accounting for42.1% (8/19), type II accounting for 57.9 % (11/19). The number of the main hepatic vein of the right , middle, and left hepatic veins were 4-10, 4410, and 3-7, respectively. Conclusion The direct CT angiography hepatic vein and portal vein in vitro can accurately display the types of hepatic veins into the in- ferior vena cava, hepatic vein types, and the distribution of the hepatic veins in the liver segment.
