摘 要:目的探讨NOD样受体-1(nucleotide-binding oligomerization domain receptor-1,NOD-1)和脆性组氨酸三位体(fragile histidine triad,FHIT)在溃疡性结肠炎组织中的表达及临床意义。方法 30例溃疡性结肠炎临床急性期及其对应临床缓解期肠镜活检标本、20例克罗恩病活检标本、20例正常肠黏膜标本,应用免疫组织化学法检测NOD-1和FHIT在不同组织中的表达并分析其临床意义。结果 NOD-1蛋白在溃疡性结肠炎急性期的阳性表达率(76.7%)高于溃疡性结肠炎缓解期(36.7%)和正常肠黏膜(20.0%)(P<0.05),与克罗恩病(80.0%)比较差异无统计学意义(P>0.05);FHIT蛋白在溃疡性结肠炎急性期阳性表达率(63.3%)高于正常肠黏膜(10.0%)、溃疡性结肠炎缓解期(23.3%)和克罗恩病(20.0%)(P<0.05)。结论 NOD-1和FHIT蛋白在溃疡性结肠炎的发生、发展中起重要作用,可提示疾病处于急性期还是缓解期,且FHIT在溃疡性结肠炎和克罗恩病的鉴别诊断中有意义。Objective To investigate the expressions of nucleotide-binding oligomerization domain receptor-1 (NOD-1) and fragile histidine triad (FHIT) in biopsy specimens of ulcerative colitis and their clinical significances. Methods The levels of NOD-1 and FHIT were detected by immunohistochemical method in 30 specimens of acute-phase ulcerative colitis and their corresponding remission-phase biopsy specimens, 20 specimens of Crohn's disease and 20 normal specimens to analyze their clinical signifieances. Results The positive expression rate of NOI)-I in the acute phase ulcerative colitis (76. 7%) was significantly higher than that in the remission-phase ulcerative colitis (56. 7%) and normal tissues (20. 0%) (P〈0. 05), and showed no significant difference in comparison with that in the specimens of Crohn's disease (80.0%) (P〉0.05). The positive expression rate of FHIT in acute-phase ulcerative colitis (63.3 %) was significantly higher than that in normal mueosa (10. 0%), remission-phase ulcerative colitis (23. 3%) and specimens of Crohn's disease (20. 0%) (P〈0.05). Conclusion NOD-1 and FHIT proteins play important roles in the development and progression of ulcerative colitis and can reveal ulcerative colitis in acute or remission phase. FHIT contributes to the differential diagnosis of ulcerative colitis and Crohn's disease.
