高压氧联合用药治疗49例椎基底动脉供血不足疗效观察

摘　　要：目的观察高压氧（HBO）联合用药治疗椎基底动脉供血不足的疗效。方法选择我院2010年1月-2014年12月确诊的椎基底动脉供血不足98例患者,在内科药物治疗基础上随机分为HBO组和对照组各49例;对照组采用内科药物治疗,HBO组在内科药物治疗的基础上加用高压氧治疗。比较两组患者的临床症状改善情况,以及高压氧治疗前后椎基底动脉平均血流速度和血液流变学变化情况。结果 HBO组与对照组比较,症状的改善更显著（95.9%vs 73.5%,χ^2=9.52,P〈0.05）。HBO组椎动脉（t=-28.60,P〈0.05）与基底动脉（t=-43.29,P〈0.05）平均血流速度较治疗前显著改善,血浆黏度（t=12.16,P〈0.05）、细胞压积（t=10.25,P〈0.05）和黏度（中切）（t=6.89,P〈0.05）较治疗前比较有显著差异。结论高压氧联合用药治疗椎基底动脉供血不足较单纯药物治疗效果更显著。Objective To observe the effect of combination therapy with hyperbaric oxygen and medication on patients with iertebrobasilar insufficiency. Methods From January 2010 to December 2014, 98 cases with iertebrobasilar insufficiency were randomly divided into HBO group（n=49） and control group（n=49）. The control group treated with medication, patients in the HBO group un dertook the combination treatment with medication and hyperbaric oxygen,Changes of clinical manifestation, the average speed of vertebrobasilar blood flow and blood rheology before and after therapy were compared between two groups. Results Patients in HBO group presented a significant improvement in clinical manifestation（95.9% vs 73.5%,χ^2=9.52,P〈0.05） when comparing with control group. After treatment, the average speed of vertebral artery blood flow（t=-28.60,P〈0.05）,basilar artery blood flow（t=-43.29,P〈0.05）, plasma viscosity（t=12.16,P〈0.05）, hematocrit（t=10.25,P〈0.05）, and dynamic viscosity（moderate shear rate）（t=6.89,P〈0.05） were significantly improved in HBO group. Conclusion The combination therapy of hyperbaric oxygen and medication on iertebrobasilar insufficiency could be more effective than medication treatment alone.
