螺旋CT对成人原发性肝肉瘤的诊断价值

摘　　要：目的 探究螺旋CT对成人原发性肝肉瘤的诊断价值。方法 回顾性分析2014年3月-2015年3月我院收治的35例疑似原发性肝肉瘤患者的临床资料,经手术病理证实成人原发性肝肉瘤患者为20例,包括未分化胚胎状肉瘤4例、腺泡状软组织肉瘤5例、淋巴瘤为3例、上皮样血管内皮瘤3例、血管肉瘤5例,探究其螺旋CT表现。结果 螺旋CT诊断成人原发性肝肉瘤的灵敏度为15.00%,特异度为46.67%,准确性为28.57%,误诊率为53.33%,漏诊率为85.00%,阳性预测值为27.27%,阴性预测值为29.17%。结论 成人原发性肝肉瘤患者的螺旋CT诊断结果和患者的病理类型具有一定的关系,对患者进行确诊时需将其螺旋CT表现和患者的病理学、免疫组织学检验结果进行综合分析。Objective To explore the diagnostic value of spiral CT in adult primary hepatosarcoma. Methods Clinical data from 35 patients diagnosed as suspected primary hepatosarcoma from March 2014 to March 2015 were retrospectively analyzed.There were 20 patients proved to be adult primary hepatosareoma by pathology after surgery including 4 cases in undifferentiated embryonic sarcoma,5 cases in alveolar soft-tissue sarcoma,3 cases in lymphoma,3 cases in epithelioid hemangioendothelioma,and 5 cases in hemangiosarcoma. Their spiral CT images were explored. Results The sensitivity,specificity,and accuracy of adult primary hepatosareoma diagnosed by spiral CT was 15.00%,46.67% ,and 28.57%.The misdiagnosis rate was 53.33% and rate of missed diagnosis was 85.00%.The positive predictive value and negative predictive value were 27.27% and 29.17% respectively. Conclusion Diagnostic results by spiral CT in patients with adult primary hepatosarcoma have some relationships with patient＇s pathological pattern.The spiral CT image,and detection outcome of pathology and immunohistology were comprehensiyely analyzed on definite diagnosis of patients.
