脑膜瘤复发患者再次手术后发生颅内感染的影响因素分析

摘　　要：目的探讨脑膜瘤复发患者实施再次手术后发生颅内感染的影响因素。方法选取2010年5月～2014年5月36例脑膜瘤复发患者进行实验研究。对所有患者的临床资料进行分析，并对其发生颅内感染的影响因素进行Logistic回归分析。结果所有患者再次手术后发生阳性感染者5例，其中金黄色葡萄球茵、铜绿假单胞茵、大肠埃希菌感染各1例，链球菌感染2例。单因素分析显示，年龄≥55岁、肿瘤恶性、合并糖尿病以及器官功能障碍、第一次手术与复发时间间隔≤6个月、术前KPS评分≤70分、此次手术前行放疗及术后实施重症监护为复发脑膜瘤再次手术患者术后颅内感染的危险因素，差异均有统计学意义（均P〈0．05）。多因素Logistic回归分析显示，合并糖尿病、第一次手术距离复发时间≤6个月及术前KPS评分≤70分为复发脑膜瘤再次手术患者术后颅内感染的独立危险因素。结论糖尿病、第一次手术距离复发时间≤6个月、术前KPS评分≤70分对复发脑膜瘤患者再次手术后颅内感染的风险较高。Objective To study the factors affecting the occurrence of intracranial infection in patients with menin- gioma recurrence after surgery. Methods 36 patients with recurrent meningioma were chosen. The influence on the oc- currence of intracranial infection factors was analyzed. Results There were 5 cases with infection after re-operation inclu- ding Staphylococcus aureus （1 case）, Pseudomonas aeruginosa （1 case）, Escherichia coli （1 case） and Streptococcus （2 cases）. 55 years of age or older, malignant tumor, diabetes, organ dysfunction, risk factors of operation and the recur- rence interval less than 6 months from the first time were the factors （P〈0.05）. Multivariate logistic regression analysis showed that diabetes mellitus, the first operation distance recurrence time less than 6 months and the preoperative KPS score less than 70 points were individual factors of intracranial infection. Conclusion Diabetes, the first operation dis- tance recurrence time less than 6 months, preoperative KPS score less than 70 points in patients with recurrent meningio- mas were high risk of intracranial infection after re-operation.
