摘 要:目的分析两种套筒冠义齿修复牙周病伴游离端缺失基牙应力分布,为牙周病牙列缺损修复方法选择提供理论依据。方法应用三维有限元方法比较分析支持型套筒冠义齿与沟槽套筒冠义齿修复牙周病伴游离端缺失基牙应力峰值及应力分布情况。结果沟槽套筒冠义齿基牙最大应力为3.05 Mpa,支持型套筒冠义齿应力峰值为3.33 Mpa,两种义齿均在远下斜向加载牙槽骨吸收达2/3时应力最大,在垂直加载牙槽骨正常时应力最小。支持型套筒冠基牙应力集中较沟槽套筒冠更偏向于牙颈部。结论沟槽套筒冠义齿基牙应力小于常规支持型套筒冠义齿,应力分布更均匀,更适合牙周病牙列缺损修复;基牙应力随着牙槽骨吸收增加而增加;非轴向力有助于基牙健康。Objective To provide theoretical basis for the selection of the restoration method for dentition defect caused by periodontal disease by analyzing the distribution of the stress of abutment between the uses of two telescope crown dentures to restore dentition defect located at distal-extension from periodontal disease.Methods Using threedimensional finite element method to make comparative analysis on peak values and distribution of the stress of abutment between regular telescope crown and groove telescope crown in distal-extension.Results The maximum stress of abutment is 3.05 Mpa in the case of using groove telescope crown denture,while the peak value of the stress of abutment for supportive telescope crown denture is 3.33 Mpa.Both the stresses of abutments are maximum values when power loaded oblique direction at the far end and periodontal tissues absorbed 2/3,while their stresses are minimum values when power loaded in vertical direction in normal alveolar bone.The concentration of the stress of abutment for supportive telescope crown is more inclined to tooth cervix than that for groove telescope crown.Conclusion The stress of abutment for groove telescope crown denture is less than that for normal telescope crown denture,and more evenly distributed,the groove telescope crown denture is more suitable for the restoration of dentition defect with periodontal disease.The stress of abutment increases along with the increased absorption of alveolar bone.the reducing oblique force benefits for the health of abutments.
