胃幽门螺杆菌感染与慢性胃炎病理变化的相关性分析

摘　　要：目的研究胃幽门螺杆菌感染与慢性胃炎的组织类型、炎症活动程度、淋巴滤泡形成、肠化等病理变化的关系。方法对我院2012年至2013年期间行胃镜检查的435例患者标本制成石蜡切片,显微镜下观察慢性胃炎的组织类型、炎症活动度、固有层淋巴滤泡形成、肠化等病理变化,同时提取DNA,用实时荧光定量PCR方法检测有无HP感染。结果 435例患者标本中,HP阳性为137例,阳性率为31.5%;其中慢性萎缩性胃炎HP阳性率为40.5%,明显高于HP阳性率为19.1%的慢性浅表性胃炎,两者比较差异有统计学意义(P<0.05);HP阳性率随炎症活动度无、轻度、中度、重度依次增高,差异有统计学意义(P<0.05);固有层淋巴滤泡形成时HP阳性率明显高于无淋巴滤泡形成的标本,差异有统计学意义(P<0.05);肠上皮化生的有无与HP感染率无明显相关性(P>0.05)。结论HP的感染与慢性胃炎的组织类型、炎症活动程度、淋巴滤泡形成等病理变化存在相关关系,当存在上述病理改变时,应高度警惕HP感染,及时进行检测和治疗。Objective To research the relationship of stomach HP infection with gastritis type,inflammation activity,lymphoid follicle formation and intestinal metaplasia.Methods observe 435 cases gastroscope biopsy from 2012to2013 by microscope.Determin gastritis type,inflammation activity,lymphoid follicle formation and intestinal metaplasia.Extract DNA and detect HP with the real-time fluorescent quantitation PCR.Results In 435 cases,137cases were HP infection positive,the positive ratio was 31.5%;Among the total,the HP positive ratio of Chronic atrophic gastritis cases was 40.5%,of chronic superficial gastritis was 19.1%.The former is significantly higher than the latter（P〈0.05）;With the aggravation of inflammation the HP positive ratio become higher and higher significantly（P〈0.05）;HP positive ratio of lymphoid follicle formation cases are significantly higher than none lymphoid follicle formation ones（P〈0.05）.Intestinal metaplasia has no significant correlation with HP infection（P〉0.05）.Conclusion HP infection has significant correlation with Chronic gastritis type,inflammation activity and lymphoid follicle formation.If there are above-mentioned pathology change,we should be care the HP infection extraordinarily,carry out detection and treatment.
