健脾活血利水方治疗肝硬化腹水的临床观察

摘　　要：目的观察健脾活血利水方治疗肝硬化腹水(脾虚血瘀证)的临床疗效。方法将纳入病例按随机数字表法分为治疗组与对照组各30例,其中对照组常规西医治疗(口服螺内酯片、呋塞米片);治疗组在对照组基础上服用健脾活血利水方,疗程均为4周,记录患者治疗前后总胆红素(TBIL)、白蛋白(ALB)化验数据、彩超及临床症状与体征的改变。结果两组治疗后总有效率方面治疗组明显优于对照组(P<0.05),在平均尿量、腹围和体质量的减少方面治疗组显著优于对照组(P<0.05),治疗组肝功能较对照组改善更为明显(P<0.05或P<0.01)。结论健脾活血利水方治疗肝硬化腹水(脾虚血瘀证)疗效明显,可减轻患者症状,并能改善患者的肝功能分级。Objective： To observe the clinical effects of Jianpi huoxue lishui Decoction on Cirrhosis ascites （spleen deficiency and blood stasis）. Methods： The cases were randomly divided into the treatment group and the control group, 30 cases in each. The control group received conventional western treatment（oral Spironolactone tablets and Furosemide tablets）. The treatment group was treated on the basis of the control group＇s treatment took Jianpi huoxue lishui Decoction. The treatment course lasted for 4 weeks. The test data of TBIL and ALB, col- or Doppler ultrasound and the change of clinical symptoms and signs were recorded before and after treatment. Results： The total effective rate of the treatment group was significantly better than that of the control group （P〈 0.05） ,as well as the average amount of urine,the reduce of abdominal circumference and body weight （P〈 0.05）. The liver function of the treatment group was significantly different compared with the control group（P〈 0.05 or P 〈 0.01）. Conclusion： Jianpi huoxue lishui Decoction has a good clinical effect on the treatment of cirrhosis as- cites （spleen deficiency and blood stasis）,and can reduce the patient＇s symptoms,and improve the patient＇s liver function classification.
