摘  要:目的探讨双相情感障碍(BD)患者丘脑-垂体-肾上腺轴(HPA)功能改变的血清蛋白质表达的差异。方法选取57例BD患者,根据有无HPA轴改变分为异常双相障碍组(异常组,22例)和正常双相障碍组(正常组,35例),采用弱阳离子交换(WCX)纳米磁珠捕获血清蛋白质组分,用基质辅助激光解吸电离飞行时间质谱(MALDI-TOF-MS)检测蛋白质质谱。结果异常组患者血清差异蛋白分子降低,在异常组与正常组之间筛选20个差异表达蛋白分子(P<0.05),8个差异蛋白峰为异常组患者特有表达。以质荷比为M4166.52、M6643.63、M6861、M10847.9的分子建立了诊断模型,其敏感性和特异性分别为86.4%和94.3%。结论双相障碍患者HPA轴功能异常,导致血清差异蛋白表达异常,可能有其特征性病理机制。Objective To explore the differences between serum protein expression profile and hypothalamus-pituitary-adrenal (HPA) axis in patients with bipolar disorder (BD). Method A total of 57 patients with BD were divided into exception group (22 cases) and normal group (35 cases) according to exception or normal function of HPA in the patients. The serum of two groups was tested with weak cationic exchange (WCX) magnetic beads for constituents of serum protein and Martrix-Assisted Laser Desorption/lionization Time of Flight Mass Spectrometry ( MALDI-TOF-MS ) for the proteomics. Results There were differences for 20 protein biomarker peaks between exception group and normal group ( P 〈 0.05 ) and specific expressions for 8 protein biomarker peaks were found in exception group. The diagnostic model was established based on these molecules with the mass-to-charge ratio of M4166.52, M6643.63, M6861 and M10847.9; it was 86.4% for the sensitivity and 94.3% for the specificity. Conclusion There are some correlations between the dysfunction of HPA and the dysfunction of serum protein expression in patients with BD and there may be characteristic pathological mechanism between them.
