摘 要:目的探讨心理互助式健康教育改善门诊首发精神分裂症患者认知功能的效果。方法将122例经门诊治疗后稳定的首发精神分裂症患者随机分为干预组(63例)和对照组(59例),干预组接受心理互助式健康教育,对照组采用常规门诊健康随访,共持续52周。两组患者分别在干预前、后给予可重复成套神经心理状态测量量表(RBANS)、威斯康星卡片分类测验(WCST)及阳性和阴性综合征量表(PANSS)评定。结果干预后干预组在RBANS总分及即刻记忆、语言功能、注意因子分差值较对照组有显著性差异(P<0.05)。干预后干预组WCST测验正确应答数、错误应答数、持续应答数差值与对照组比较有显著性差异(P<0.05)。RBANS各因子均与PANSS阴性症状因子分呈负相关(P<0.05);而RBANS除视觉广度以外的各因子与PANSS阳性症状因子分呈负相关(P<0.05)。结论门诊首发精神分裂症患者的精神症状与认知功能损害有相关性,心理互助式健康教育可以改善门诊首发精神分裂症患者的认知功能损害。Objective To explore the efficacy of mutual psychologically supportive health education to improve cognitive function of outpatients with first-episode schizophrenia. Methods A total of 122 outpatients with first-episode schizophrenia were randomly divided into study group (63 cases ) treated with mutual psychologically supportive health education for 52weeks and control group( 59 cases) treated with routine follow-up for 52 weeks. They were all assessed with Repeatable Battery for the Assessment of Neuropsychological Status(RBANS) , Wisconsin Card Sorting Test(WCST) and Positive and Negative Syndrome (PANSS) before and after invention. Results After invention, the total score and factor scores of immediate memory, language function and attention in RBANS in study group were all significantly different from those in control group ( P 〈 O. 05 ). After invention, there were significant differences between study group and control group in total correct responses, total errors and preservative responses in WCST ( P 〈 O. 05 ). All factor scores in I^BANS were negatively related to the negative factor scores in PANSS( P 〈0.05 ) , and all factor scores except the factor of visual span in RBANS were negatively related to the positive factor scores in PANSS( P 〈 O. 05 ). Conclusion The psychiatric symptoms in outpatients with first-episode schizophrenia are related to their impairment of cognitive function and it' s effective for mutual psychologically supportive health education to improve their cognitive function.
