改良的人工晶体眼硅油取出术探讨

摘　　要：目的探讨改进型透明角膜三切口方法,表面麻醉下,利用前房灌注技术,对玻璃体切除术后的人工晶状体硅油填充眼,经瞳孔行硅油取出的有效性与安全性。方法 21例玻璃体切除术后人工晶状体硅油填充眼患眼,在行硅油取出时,表面麻醉下,采用颞下方透明角膜隧道切口,23G灌注套管连接灌注液进入前房,保持连续灌注,并做透明角膜辅助切口,伸入T钩拨开人工晶体,使其偏向一侧,露出人工晶体与虹膜间隙,从上方透明角膜隧道切口经瞳孔主动抽吸硅油。结果 21眼均一次性完成硅油取出,手术过程顺利,前房稳定,手术切口不须缝合,术后19眼视力提高。结论改进的人工晶体硅油填充眼表面麻醉下经瞳孔行硅油取出具有损伤小、安全有效的特点。Objective To investigate the effect and safety of silicone oil filled eyes with silicone oil filled eyes under the surface anesthesia, the improved type of transparent corneal incision was performed. Methods 21 cases of silicone oil filled eyes with intraocular lens after vitrectomy, when taken out of silicone oil line, surface anesthesia, using the temporal below a clear corneal tunnel incision, 23 G into anterior chamber perfusion casing connection fluids, keep continuous infusion, and do transparent cornea auxiliary incision, into T hook through artificial crystal, make its tilted away, with artificial lens and iris clearance, from above a clear corneal tunnel incision through the pupil active suction silicone oil. Results 21 eyes are completed a one-time silicone oil, the procedure smoothly, stable anterior chamber, incision not suture, in 19 eyes vision improved. Conclusion This kind of improved artificial crystal silicon oil fill the eye through the pupil silicone oil was removed under surface anesthesia has the characteristics of small injury, safe and effective
