卵巢成熟性囊性畸胎瘤合并类癌2例临床病理观察

摘　　要：目的探讨卵巢成熟性囊性畸胎瘤合并类癌的临床病理特征。方法观察和分析2例卵巢成熟性囊性畸胎瘤合并类癌病例的病理组织学及免疫表型的特点,并结合相关文献进行讨论。结果 2例卵巢类癌均伴有成熟性囊性畸胎瘤成分,例1为妊娠合并卵巢畸胎瘤,类癌成分呈岛状、巢团状生长;例2畸胎瘤合并卵巢甲状腺肿类癌,镜下甲状腺组织与类癌相互交织,类癌呈梁索状、岛状生长;2例瘤细胞核呈典型"胡椒盐样",染色质细腻,核分裂不明显。免疫组化:Cg A、Syn、CD56和CK均(+);例2类癌区CK19(+),TTF-1(-)。结论病理形态学观察和免疫组化染色有助于明确诊断。卵巢类癌预后较好。Objective To study the clinicopathological features of ovarian carcinoid with mature cystic teratoma.Methods Ovarian carcinoid with mature cystic teratoma were observed with gross,microscopic and immunohistochemical stains and discussed with literature review.Results The two ovarian carcinod tumors were associated with mature cystic teratoma.The first case was pregnancy complicated with ovarian insular carcinoid,island shaped,nest like-growth.The second case was strumal carcinoid of the ovary,On light microscopy,the tumors were composed of thyroid tissue and carcinoid component.Ovarian carcinoid showed trabecular and insular growth.The nuclei of both tumor cells showed typical ＂salt and pepper ＂,fine chromatin,mitosis is not obvious.Immunohistochemcally,they were positive for chromogranin A,Synaptophysin,CK and CD56.Strumal carcinoid area showed CK19 positive and TTF-1 negative.Conclusion The diagnosis of ovarian carcinoid with mature cystic teratoma can be established on histopathologic,immunohistochemical and electron microscopic studies.The prognosis of patients with ovarian carcinoid is favorable because these tumors are not associated with metastases.The optimal treatment is the excision of the affected adnexa,which results in a complete cure.Follow-up of 17 and 5 months shows no recurrence and metastasis.
