摘　　要：目的探讨原发性恶性血管球瘤(MGT)的临床病理特征、诊断与鉴别诊断及治疗与预后特点。方法对1例原发于椎管内的恶性血管球瘤进行光镜观察、免疫组化染色,并结合文献分析讨论。结果肿瘤由小圆形细胞和短梭形细胞组成,瘤细胞围绕裂隙状血管弥漫生长。瘤细胞大小较一致,胞质较少、嗜酸性,胞膜界清,核深染,有异型,非典型核分裂象多见(>10个/10HPF),可见出血和坏死。免疫组化:肿瘤细胞SMA、H-caldesmon、vimentin和Ⅳ型胶原(+)。结论椎管内原发性恶性血管球瘤是罕见的恶性肿瘤,其组织学形态常常与其他恶性肿瘤相似,确诊必须依靠临床特点、组织学形态及免疫组化结果综合分析。Objective To explore the morphological features,diagnosis,differential diagnosis,therapy and prognosis of the primary malignant glomus tumor.Methods A case of primary malignant glomus tumor in spinal canal was studied with the help of light microscopy,mmunohistochemistry,with review of the relevant literature.Results The tumor was composed of small round sells and spindle shaped cells.The tumor cells around the slit shaped vascular grew diffusely.The size of the tumor cells was more consistent,less cytoplasm,eosinophilic,marked nuclear atypia and any level of mitotic activity.Immunohistochemically,there was positive reactivity for SMA,h-caldesmon and vimentin.Conclusion Primary malignant glomus tumor in spinal canal is rare.We must grasp the histological characteristics and clinical history of the tumor and comprehensively immunohistochemical are also essential.
