摘  要:目的 研究垂体微泌乳素瘤的MRI平扫和动态强化表现。方法 收集经临床和MRI扫描确诊的垂体微泌乳素瘤67例患者均使用Philips 1.5T双梯磁共振机进行MRI平扫,并进行冠状位3 mm层厚、13 s间隔、15个动态强化扫描。结果 肿瘤平均直径4.37 mm,位于垂体前叶左侧26例,右侧28例,垂体柄下部13例。肿瘤呈类圆形26例,不规则形41例。MRI平扫在T1WI肿瘤呈低或稍低信号42例(62.69%),高信号1例(1.49%),等信号24例(35.82%);在T2WI呈高或较高信号36例(53.73%),等信号19例(28.36%),低信号12例(17.91%)。MRI动态强化扫描在1 min 5 s-1 min 31 s肿瘤异常强化明显,呈低强化者53例(79.10%),轻度不均匀强化者5例(7.46%),中度均匀强化者7例(10.45%),明显强化者2例(2.99%)。结论 MRI平扫结合动态强化扫描对诊断垂体微泌乳素瘤有重要作用。Objective To study the findings of pituitary microprolactinoma on conventional MRI and dynamic enhancement MRI. Methods 67 cases of pituitary microprolactinomas proved by clinic and MRI scan were presented. Philips Achieva 1.5T dual gradient MRI system was used to conventional scan and enhanced dynamic scan on coronal section with 3 mm thickness, 13 s intervals and 15 times of dynamic enhancement images in all patients. Results The average diameter of pituitary microprolactinoma was 4.37 mm. 26 cases were located in the left anterior lobe, 28 in the right anterior lobe and 13 in the lower part of pituitary stalk. The tumors were showed in the shape of round-like in 26 cases and irregular in 41 cases. On conventional T1WI,low signal intensity of the lesion was found in 42 cases ( 62.69 %), high signal intensity in 1 case ( 1.49 % ) and iso-signal intensity in 24 cases (35. 82% ), respectively. On con ventional T2 WI,high signal intensity was found in 36 cases (53. 73 %), iso signal intensity in 19 cases (28.36 % ) ,and low signal intensity in 12 cases ( 17.91% ), respectively. On enhanced dynamic MRI, abnormal enhancement was showed between 1 min 5 s- 1 min 31 s after injection of contrast,low enhancement was demonstrated in 53 cases (79. 10%) ,high enhancement in 2 cases (2.99% ), mild inhomogeneous enhancement, in 5 cases (7.46 % ) ,and moderate homogenous enhancement in 7 cases ( 10.45 % ), respectively. Conclusion Conventional MRI combined with enhanced dynamic MRI is significant in diagnosis of pituitary microprolactinoma.
