摘 要:目的探讨影响有晶状体眼后房型人工晶状体(ICL)植入术后影响拱高的相关因素。方法行ICL植入术的24例(47只眼)高度近视的患者,术前检查资料及术后6个月随访资料,包括性别、年龄、裸眼视力、最佳矫正视力、等值球镜度数(D)、眼轴长度、前房深度(ACD)、白到白、角膜厚度、眼压及超声生物显微镜UBM检查并进行相关分析,UBM的测量数据包含巩膜突间距(SSD)、房角开放距离(AOD500、小梁虹膜夹角(TIA)、周边虹膜厚度(IT500)、虹膜膨隆度(IC),术后6个月(OCT)所测得的拱高进行相关分析。结果手术后所有患者裸眼视力与术前相比较都有显著提高。术后6个月患者裸眼视力,51%(24/47)≥1.0,74.5%(35/47)≥0.8,93.6%(44/47)≥0.5。眼压,手术前后差异有统计学意义,术后比术前稍有增高,但均在正常范围内。手术前后AOD500、ACA500、IC、SSD比较,差异均有统计学,术后均有明显减小,但在正常范围内。采用Pearson积差相关分析结果显示术后6个月拱高与所植入人工晶体长度呈正相关性(r=0.307,P<0.05),用直线回归方程描述为:y=0.006x+0.517,标准化回归系数为0.087,即人工晶体长度每变化1 mm,可引起6μm拱高的变化;术后6个月拱高与术前AOD500、TIA、IT500、IC无相关性(P>0.05),而与术前ACD、WTW呈正相关(r分别为0.544、0.414,P<0.05),用直线回归方程描述分别为:y=0.016x+0.402和y=0.238x-2.258,标准化回归系数分别为0.082、0.414,即术前测量的ACD每变化1 mm,可引起16μm拱高的变化;术前测量的WTW每变化1 mm,可引起0.238 mm拱高的变化。结论 ICL植入术后前房角AOD500、TIA、SSD、IC均有明显的变化,但均在正常范围内。ICL植入术后的拱高与术前ACD、WTW和人工晶体长度呈正相关性,过高的拱高均出现在白到白的距离比较大且前房也较深的患者,过低的拱高均出现在白到白距离偏小和前房较浅的患者。因此,在临床上对这些患者在选择ICL长度�Objective To explore the effect on valut after Implantable Collamber Lens (ICL) implanted for high myopic. Methods 47 eyes of 24 patients were performed implantation of ICL (model V4, Staar Surgical). Preoperative information were meas- ured, it including sex, age, uncorrected visual acuity, best corrected visual acuity, intraocular pressure, corneal endothelial cell count and white - to - white. Ultrasound biomicroscopy was used to measure the deepness of anterior chamber ( ACD ) , trabecular - iris angle (TIA) ,angle opening distance 500 microns from the scleral spur (AOD500) ,peripheral iris thickness (ITS00) ,iris convex (IC) at point 3,6,9, and 12 respectively. The repeated measurement anova statistical method was used to analyzed statistical data by using SPSS 19. Results The postoperative VA was significantly improved compared with preoperative VA in all patients. After six mouths of follow- ing - up ,51 percent (24/47) of patients UCVA were greater than 1.0,74.5 percent (35/47) of patients UCVA were greater than 0.8, 93.6 percent (44/47) of patients UCVA were greater than 0.5. The postoperative intraocular pressure was increased than that in preop- erative ( P 〈 0.05 ). The postoperaive SSD, AODS00 and ACA500 value were decreased obviously compared to preoperative ( P 〈 0.05 ). The valut was no correlation with AOD500, ACA500, IT500 and IC ( P 〉 0.05 ). The influential factors of value included preoperative ACD, white - to - white, the length of lens ( P 〈 0.05 ). Conclusion The structure of anterior chamber and the influential factor of vault is researched in this study and the long - term follow - up and large sample size for further clinical study is still required.
