泪道支架植入在内窥镜下鼻腔泪囊吻合中的应用

摘　　要：目的探讨泪道支架植入在内窥镜下鼻腔泪囊吻合术中的应用效果。方法慢性泪囊炎患者76例随机分为治疗组与对照组各38例,对照组采用传统泪道支架扩张术,治疗组采用内窥镜下泪道支架植入术,观察两组的手术成功率、治疗有效率及并发症发生情况。结果治疗组的手术成功率为94.7%,显著高于对照组的78.9%(P<0.05)。两组手术失败的原因主要为术中出血与吻合口肉芽组织堵塞。术后1个月进行判定,治疗组的总有效率为97.3%,显著高于对照组的84.2%(P<0.05)。治疗组术后1个月的支架阻塞、泪道扩张管移位、溢泪、泪囊区硬物感等并发症发生情况明显少于对照组(P<0.05)。结论泪道支架植入在内窥镜下鼻腔泪囊吻合术中的应用,可提高手术成功率,减少术后并发症的发生,促进总体疗效的提高。Objective To investigate the application effects of lacrimal stent implantation on endoscopic dacryorhinocystostomy. Methods 76 patients with chronic dacryocystitis were randomly and equally divided into treatment group and control group. The control group was given traditional lacrimal duct stent dilatation, while the treatment group was treated by endoscopic lacrimal duct stent implantation. Observation was made in the operation success rate, the effective rate of treatment, and complications between the two groups. Results The success rate of the treatment group was 94.7% and higher than that of the control group （78.9%） （P 〈 0.05）. The main reasons for the operation failure were perioperative bleeding and granulation tissue blockage in anastomotic stoma. One month after the operation, the total effective rate of the treatment group was 97.3% and significantly higher than that of the control group （84.2%） （P 〈 0.05）. The complications like stent occlusion, lacrimal duct dilatation tube shifting, epiphora, and feeling of hard objects in lacrimal sac area in the treatment group were significantly less than those in the control group one month after the operation （P 〈 0.05）. Conclusion The application of lacrimal stent implantation to endoscopic dacryorhinocystostomy can increase the operation success rate, reduce the incidence of postoperative complications, and promote the improvement of the overall efficacy.
