彩色多普勒超声联合弹性成像在三阴性乳腺癌患者诊断中的应用

摘　　要：目的:探讨彩色多普勒超声联合超声弹性成像(UE)在三阴性乳腺癌(TNBC)患者临床诊断中的应用价值。方法:应用彩色多普勒超声联合弹性成像技术对39例三阴性乳腺癌患者(设为TNBC组)和177例非三阴性乳腺癌患者(设为非TNBC组)的超声声像图特征进行分析比较。结果:1二维超声表现,TNBC组患者缺乏如形态不规则,边界不清晰,边缘毛刺征、微钙化、后方回声衰减等乳腺癌典型的二维超声表现,难与良性肿块进行鉴别,但其淋巴结转移率较高;2彩色多普勒超声,两组患者均具有丰富的血流信号,而且RI高;3两组患者的UE评分无显著差异。结论:彩色多普勒超声联合弹性成像应与二维超声结合,综合评价有助于提高对三阴性乳腺癌患者的诊断水平。Objective:To explore diagnostic value of color Doppler ultrasound combined with ultrasonic elastography ( UE) in triple negative breast cancer ( TNBC) . Methods: The color Doppler ultrasound and UE image characteristics of TNBC patients were analyzed and compared with those of non-TNBC patients. Results:(1) In two-dimensional ultrasonography, TNBC group lacked typi-cal features of the malignant tumor ( for example, irregular shape, indefinite border, spicules of margin, microcalcifications, posterior echo attenuation, et al) . Therefore, it was difficult to distinguish TNBC from benign tumors, but TNBC patients had higher lymph node metastasis rate. (2) The color Doppler imaging of the two groups showed rich blood supply signals and high resistance index. (3) In UE imaging, there was no significant difference between the two groups. Conclusions:Comprehensively evaluation of color Doppler ul-trasound combined with UE will raise the diagnosis level of TNBC.
