高脂血症性急性胰腺炎降脂治疗疗效分析

摘　　要：目的分析低分子肝素和胰岛素在治疗高脂血症性急性胰腺炎(HLAP)过程中降低高甘油三酯血症的临床疗效。方法将HLAP病例14例作为研究对象,通过营养支持、抑酸、抑制胰酶、脏器功能支持、预防感染等急性胰腺炎的治疗和"葡萄糖+胰岛素+低分子肝素"降脂治疗,观察甘油三酯、胆固醇变化趋势、平均住院日以及疾病转归。结果 14例患者中13例痊愈出院,平均住院日11.5天,患者治疗前甘油三酯平均浓度43.7 mmol/L,治疗后12小时甘油三酯平均浓度降至25.9 mmol/L,24小时降至13.8 mmol/L,36小时降至10.9 mmol/L(低于11.3 mmol/L),96小时降至3.6 mmol/L(低于5.65mmol/L);胆固醇浓度也有所下降。结论低分子肝素和胰岛素联合治疗对于降低血甘油三酯浓度具有显著的效果。Objective To analyze the efficacy of hyperlipidemic acute pancreatitis（ HLAP） treatment with low molecular heparin and insulin. Method Fourteen patients with HLAP at our hospital from July 2013 to July 2015 were treated with glucose,insulin and low molecular heparin on the basis of nutritional support,control acid and pancreatin,organ function support and infection prevention. We collected the data of changes in triglycerides and cholesterol,hospitalization time and prognosis of disease. Result Of 14 cases,13 cases recovered. Average length of stay in hospital 11. 5 days. The average concentration of triglyceride in patients before treatment was 43. 7 mmol / L. After treatment,the average concentration of triglyceride decreased to 25. 9mmol / L after 12 hours,24 hours to13. 8mmol / L,36 hours to 10. 9mmol / L（ lower 11. 3 mmol / L）,96 hours to 3. 6mmol / L（ lower 5. 6mmol / L）. Cholesterol levels also decreased. Conclusion Combination therapy with low molecular heparin and insulin has a significant effect on lowering blood triglyceride concentration.
