两种材质缓冲型套筒冠义齿修复牙列缺损的临床研究

摘　　要：目的研究两种材质缓冲型套筒冠义齿修复牙列缺损的临床效果。方法选择38例慢性牙周炎患者经牙周治疗后用套筒冠义齿修复牙列缺损,共制作套筒冠义齿59件,其中30件采用金沉积内外冠(A组)29件采用氧化锆全锆内冠、金沉积外冠(B组)。义齿戴用后6、12、18、24、30个月复诊,由医生结合口内检查结果和患者主观感受,对患者戴用修复体后的临床效果进行评价。结果 38例患者戴用义齿30个月时,主观满意率为97.35%。2组患者的基牙牙周指标在观察期内均保持稳定(P>0.05)。义齿使用18个月后,有3件金沉积内外冠套筒冠义齿出现固位力下降,而采用氧化锆全锆内冠加金沉积外冠套筒冠义齿修复的患者未出现固位问题。结论套筒冠义齿对牙周病伴牙列缺损的患者具有良好的修复效果,采用氧化锆全锆内冠加金沉积外冠设计的套筒冠义齿经长期使用后,固位效果优于金沉积内外冠套筒冠义齿。Objective To investigate the clinical effect of two different alloy telescopic dentures in the dentition defect treatment.Methods 38 patients with chronic periodontal diseases were enrolled after comprehensive periodontal treatment.59 telescopic crown dentures were made for the defective dentition,in which 30 pairs of inner and outer crowns made of gold alloy,and 29 pairs of inner crows made of zirconia and outer crowns made of gold alloy.The follow-up period varied from 6 to 30 months.The prosthesis effect was evaluated through the clinical examination and the questionnaires from the patients.Results The clinical periodontal index of the abutment teeth were stable in 30-months＇ follow-up,and there was no statistical difference between 2 groups.In 18-months＇ follow-up,the denture-retaining forces decreased in 3 telescope dentures which made of gold alloy,while in other group,all of the telescope dentures with inner crows made of zirconia and outer crows made of gold alloy were stable.Conclusion Telescope dentures is effective in prosthodontic treatment of defective dentition.The denture-retaining forces of telescope dentures with inner crows made of zirconia and outer crows made of gold alloy are better than telescope dentures made of gold alloy.
