子宫肌瘤实施子宫切除术临床路径研究

摘　　要：目的分析评价子宫肌瘤子宫切除术临床路径在临床应用中的效果。方法收集2013年实施临床路径病例103例和同期未实施临床路径的患者103例,应用统计描述、t检验、X2检验和秩和检验等方法进行统计学分析。结果实施临床路径组在住院天数、术前日比未实施路径组有所降低;住院总费用、药费、化验费、床位费和其他费用路径实施组也低于未实施路径组;两组在住院天数、住院总费用、药费、床位费和其他费用方面比较有统计学意义(P<0.05);在医疗服务评价中,临床路径实施组的科室总体满意度、科室医务人员满意度和科室护理人员满意度均高于未实施临床路径组,两组比较有统计学意义(P<0.05)。结论实施临床路径可以缩短住院天数,降低医疗费用,规范医护主动医疗行为,提高医院及科室的工作效率,提高患者满意度,缓和医患关系,使医患者双方受益。Objective To analyze the effect of clinical Pathway of myoma of uterus hysterectomy in clinical.Methods Collecting 103 patients of myoma who received uterus hysterectomy and 103 patients of myoma who not received uterus hysterectomy during 2013. The general descriptive statistics, T-test, chi-spuare test and rank sun test methods were used to assess the effect of myoma of uterus hysterectomy pathway. Results After comparison of the cases who were treated by myoma of uterus hysterectomy pathway with a control group of the study indicates that the numbers of in-hospital days, the days between admission and operation reduced in the group of implementation; The average cost of the total expense in hospital, the cost of drug, the cost of surgical operation, the cost of examination, the cost of accommodation and the other costs also were reduced in the group of implementation. There were the significant statistical differences in the index of the days in hospital, the total expense in hospital, the expense of drug, the expense of bed and the other expense（P〈0.05）;In assess of medical satisfaction, the scores of office staff satisfaction, the doctor satisfaction and the nurse satisfaction were high in the implemented group than in the control group. There were significant statistical difference in the index of office staff satisfaction, the doctor satisfaction and the nurse satisfaction（P〈0.05）. Conclusion The present study shows that the implementation of myoma of uterus hysterectomy pathway can reduce the average days of stay in hospital, reduce the average cost of hospitalization,standardize clinical treatment and nursing behavior effectively, reduce the repeat work, increase the work efficiency of hospital, increase the patients satisfaction, alleviate doctor-patient relationship and benefit the doctor and patients.The myoma of uterus hysterectomy pathway can bring more economical and social benefits for the hospital.
