上切牙内收前后的牙根吸收研究

摘　　要：目的调查上切牙内收前后的牙根吸收状况,并对该过程中牙根吸收的影响因素进行初步的探讨。方法选择上颌需要拔除双侧第一前磨牙且需要强支抗内收上切牙的患者46例,分别于上切牙内收前(T1)和上切牙内收后(T2)拍摄上颌切牙的平行投照根尖片和头颅侧位片,通过测量评价,得到每颗切牙的牙根吸收量和上中切牙的角度、位置及其变化量,并记录其它诊断和治疗因素。统计上切牙的牙根吸收量及吸收率,并对各因素作多因素分析。结果 1上切牙内收前后,中切牙的牙根吸收量平均为1.14mm(SD 0.81mm),侧切牙为1.23mm(SD 0.72mm)。2中切牙的牙根吸收率平均为8.18%(SD 6.26%),侧切牙为8.57%(SD 5.17%)。3多元线形回归分析表明成人患者、切牙内收疗程长、内收后切牙根尖距离腭侧骨皮质近、种植体支抗的使用增加了内收前后中切牙牙根吸收的程度。结论上切牙内收前后有较为明显的牙根吸收。我们的研究因素中存在此阶段与上中切牙牙根吸收相关的影响因素。Objective To investigate root resorption of the maxillary incisors during retraction stage and to explore the possible risk factors. Methods Forty-six patients treated with the bilateral maxillary first premolars extractions and high anchorage to retract the maxillary incisors were selected. The periapical radiographs of the maxillary incisors using the paralleling technique and the cephalograms were obtained before retraction（T1） and after retraction（T2）. Root resorption value of each incisor were measured. The inclination angle,the position of the maxillary central incisors and the changes during retraction were measured. Other diagnosis and treatment parameters were also recorded.Descriptive statistics of the root resorption value and rate, multivariate analysis were taken. Results （1）During maxillary incisors retraction, the average root resorption values were 1.14mm（SD 0.81mm）for the central incisors and 1.23mm（SD0.72mm）for the lateral incisors. （2） The average root resorption rates were 8.18%（SD 6.26%）for the central incisors and8.57%（SD 5.17%） for the lateral incisors. （3）Multivariate linear regression showed that adults, increased retraction time,more proximity from the root apices to the palatal cortical bone surface after retraction, use of implant as anchorage increase the risk of root resorption of the central incisors. Conclusion A notable value of root resorption was found for the maxillary incisors during retraction stage. Some risk factors were found for the root resorption of the maxillary central incisors during retraction.
