胃蛋白酶原与胃癌单克隆抗体联合检测在胃癌前病变诊断中的应用

摘　　要：目的:探讨血清胃蛋白酶原(pepsinogen,P G)与胃癌单克隆抗体(MG-Antigen7-Ag,MG7-Ag)联合检测在胃癌前病变诊断中的应用价值.方法:对因上消化道症状就诊于天津市中医药研究院附属医院,并行胃镜和病理检查的102例患者,依据结果分为胃癌前病变组65例和胃良性病变组37例,另取体检中心健康体检者30例作为正常对照组.对3组人员进行胃蛋白酶原Ⅰ(PGⅠ)、胃蛋白酶原Ⅱ(PGⅡ)及PGR和胃癌单克隆抗体(MG7-Ag)检测.得出结果后结合胃镜及病理综合分析两项血清学指标与胃黏膜状态的相关性.结果:与正常对照组和胃良性病变组比较,胃癌前病变组血清PGⅠ、PGR水平均明显降低(P<0.05),而PGⅡ和MG7-Ag水平明显增高(P<0.05).PG、MG7-Ag联合检测胃癌前病变的灵敏度为92.3%,高于单项肿瘤标志物检测的敏感度;其特异性为90.0%,较单项指标无明显差别.结论:PG与MG7-Ag联合检测对胃癌前病变的诊断敏感性高,特异性佳,可以作为胃癌前病变筛查的良好指标,值得向临床进一步推广.AIM：To assess the value of combined detection of serum pepsinogen（PG） and MG-Antigen7-Ag（MG7-Ag） in the diagnosis of precancerous lesions of gastric cancer（PLGC）.METHODS：The study included 102 patients with digestive tract symptoms who sought medical treatment at Tianjin Academy of Traditional Chinese Medicine Affiliated Hospital.According to endoscopic and pathological results,the patients were divided into two groups,65 patients with PLGC and 37 with benign lesions.Additionally,30 healthy volunteers were included in a normal control group.These three groups were subjected to PG I,PGⅡ,PGR and MG7-Ag detection tests.Using endoscopic and pathology results as a standard,the correlation of PG and MG7-Ag with gastric lesions was determined.RESULTS：Compared with the normal control group and benign gastric lesion group,serum PG I and PGR levels were significantly reduced（P〈0.05）,and PGⅡ and MG7-Ag levels were significantly increased（P〈0.05） in the PLGC group.The sensitivity of combined detection of serum PG and MG7-Ag in the diagnosis of PLGC was 92.3%,which was significantly higher than those of single tumor marker（TM） detection;and the specificity was90.0%,which was not significantly different from those of single TM detection.CONCLUSION：The combined detection of PG and MG7-Ag in the diagnosis of PLGC has high sensitivity,good specificity,and can be used as a good index for screening of PLGC.
