医用生物蛋白胶联合自体结膜移植及治疗性接触镜配戴治疗单纯翼状胬肉

摘　　要：目的评价医用生物蛋白胶联合自体结膜移植加治疗性接触镜配戴治疗单纯性翼状胬肉临床疗效。方法36例(46只眼)患单纯性翼状胬肉患者分为2组,观察组16例22只眼行翼状胬肉切除联合医用生物蛋白胶应用及自体结膜移植加治疗性接触镜配戴,对照组20例24眼行翼状胬肉切除联合自体结膜移植缝线固定术。比较2组患者手术时间与术后1、7 d和1个月时异物感,3 d时上皮愈合情况及1 a复发情况。结果观察组和对照组患者手术时间分别为(18.23±2.29)min和(27.04±5.16)min,观察组患者手术时间短于对照组(t=10.674,P<0.05)。观察组和对照组治疗后有明显异物感患者在术后1 d分别有8眼(36.4%)和21眼(87.5%),术后7 d分别有2眼(9.1%)和11眼(45.8%),观察组患者术后1、7 d明显异物感发生率均低于对照组(χ2=12.883、7.643,P<0.05)。观察组和对照组患者在术后3 d角膜上皮未愈合者分别有2眼(9.1%)和11眼(45.8%),观察组患者角膜上皮未愈合率低于对照组(χ2=7.643,P<0.05)。观察组和对照组术后1 a复发患者分别为0眼和3眼,差异无统计学意义(P>0.05)。结论翼状胬肉切除联合医用生物蛋白胶及自体结膜移植加治疗性接触镜配戴治疗单纯性翼状胬肉与单纯联合自体结膜移植相比,可明显提高患者术后早期的舒适度,加快角膜创面恢复和缩短手术时间。Objective To investigate the clinical effectiveness of conjunctival autografting combined with fibrin glue and usage of soft contact lens（ SCL） for treatment of pterygium. Methods Totally thirty-six patients（ 46 eyes） with pterygium were selected and divided into observer group and control group. The patients in observation group（ 16 patients,22 eyes） were performed with pterygium excision operation combined with fibrin glue application,conjunctival autograft and therapeutic SCL;the patients in control group（ 20 patients,24 eyes） were performed with pterygium excision combined with conjunctival autografting operation. The operation time,foreign body sensation at 1 day,7 days and 1 month after operation,epithelium healing condition and recurrence in 1 year were compared between the two groups. Results The operation time in observation group and control group was（ 18. 23 ± 2. 29） min and（ 27. 04 ± 5. 16） min,respectively; the operation time of patient in observation group was significantly shorter than control group（ t = 10. 674,P〈0. 05）. Eight eyes（ 36. 4%） in observation group and twenty-one eyes（ 87. 5%） in control group appeared foreign body sensation at 1 day after treatment; two eyes（ 9. 1%） in observation group and eleven eyes（ 45. 8%） in control group appeared foreign body sensation at 7 days after treatment; the incidence of foreign body sensation of patient in observation group was significantly lower than that in control group at 1 day,7 days after treatment（ χ2= 12. 883,7. 643,P〈0. 05）. Corneal epithelial of two eyes（ 9. 1%） in observation group and eleven eyes（ 45. 8%）in control group without healing at 3 days after the operation; there was statistic difference of corneal epithelial non healing rate between the two groups（ χ2= 7. 643,P〈0. 05）. None eye recurred in observation group and three eyes recurred in control group in one year after treatment; there were no statistic difference of the recurrence rates between the two groups（
