上唇滑行Z形瓣即刻修复下唇癌切除后缺损的临床观察

摘　　要：目的:观察下唇癌扩大切除后应用滑行Z字瓣修复缺损的临床疗效。方法:下唇鳞状细胞癌6例,其中男性5例,女性1例。原发灶宽度均接近或达到下唇宽度的1/2,术式均采用原发灶边缘外1cm扩大切除,上唇及颊部滑行Z形瓣转移修复,术中保留面动脉和面前静脉分出向皮瓣走形的分支。术后6个月和8个月随访,观察上下唇形态和功能。结果:除1例下唇正中因张力大裂开小部分外,其他患者手术面部创口愈合良好,形态较为满意。结论:上唇及颊部滑行Z形瓣转移修复下唇缺损是一种较为理想的修复方法,可取得较为理想的外形和功能修复。Objective： To observe the clinical effects of Z-shaped sliding flap to repair the defects due to lower lip carcinoma resection. Methods： 6 cases（5 male and one female） with lower lip squamous cell carcinoma were recruited in this study.The lesion width reached or exceeded 1/2 of the lower lip width. Extended resection of the lesion and Z-shaped flap sliding from upper lip and buccal soft tissue were applied to all cases. The blood vessel branches from facial artery and upper facial vein were preserved to supply the flap. Evaluation of the morphology and function of lips was performed respectively after 6 months and 8 months. Results： All flaps healed well and achieved satisfactory shapes except one flap cracked in the lower lip due to tension. Conclusion： Application of Z-shaped sliding flap to repair the lower lip defects leads to ideal appearance and functional restoration.
