摘 要:目的探讨锥束CT(CBCT)三维(3D)重建技术在上颌埋伏阻生前牙外科导萌正畸术中的应用价值。方法对16例(20颗)上颌埋伏阻生前牙患者进行CBCT 3D重建,应用SIMPLANT软件手术设计平台进行埋伏牙定位,测量和手术模拟,根据计算机辅助设计的结果行外科手术导萌联合正畸牵引。结果 CBCT 3D重建图像能清楚地显示埋伏牙位置、倾斜角度、牙冠方向、牙根形态及其与邻牙牙根和周围组织的关系;16例(20颗)患牙能够基本依据计算机模拟设计的步骤来完成手术。结论 CBCT 3D重建技术能精确指导手术开窗的入路和牵引装置的固定,有效降低手术的风险和难度。Objective To investigate the clinical value of three-dimensional(3D) reconstruction of cone-beam CT(CBCT) in the surgical exposure of anterior maxillary impacted teeth during orthodontic treatment. Methods The 3D reconstruction of CBCT was performed in 16 cases(20 teeth), whose anterior maxillary teeth were impacted: Computer software SIMPLANT was applied to locate and measure the impacted teeth and simulate the surgical exposure. Surgical orthodontic treatment was applied to guide the impacted maxillary teeth eruption according to the results of SIMPLANT. Results The 3D reconstruction of CBCT could display the impacted teeth's location, angle of inclination, direction of crowns, and relationship with adjacent teeth and surrounding tissues. With the simulation by the SIMPLANT, the surgical procedure was performed with complete exposure of 16 cases (20 teeth). Conclusion With 3D reconstruction, C]3CT technique may precisely guide surgical approach and fixation of traction apparatus and effectively reduce the risk and difficulty of orthodontic treatment for impacted teeth.
