摘  要:目的检测体外受精-胚胎移植(In vitro fertilization-embryo transfer,IVF-ET)患者不同大小卵泡中肝素结合表皮生长因子(Heparin-binding epidermal growth factor,HB-EGF)的表达水平,探讨HBEGF与相应卵泡液中雌二醇(estraadid,E2)、孕激素(progesterone,P)及E2/P比值之间的相关性及其意义。方法选取2014年4~9月哈尔滨医科大学第一临床医院生殖中心因男方因素或者输卵管因素行辅助生殖治疗的58例不孕患者的临床资料,采用酶联免疫吸附法(Enzyme-linked immuno sorbent assay,ELISA)检测因输卵管因素或者男方因素行辅助生殖治疗患者卵泡液中HB-EGF的表达情况,电化学发光法(electrochemiluminescence,ECLIA)检测该卵泡液中雌孕激素水平。大卵泡组(32例)和小卵泡组(26例)互为对照组。结果 ELISA法检测发现HB-EGF在卵泡液中有表达;卵泡液中HB-EGF、E2和E2/P与卵泡大小呈正相关性关系,HB-EGF、E2和E2/P在大卵泡组水平显著高于小卵泡组(P〈0.05),但P与卵泡大小无相关性(P=0.226)。大卵泡组中,HE-EGF与E2和E2/P具有正相关性,相关系数为0.562和0.487,即HB-EGF越大,E2和E2/P越大,与P不具有相关性(P=0.335)。在小卵泡组中,HE-EGF与E2和E2/P具有正相关性,相关系数为0.454和0.728,与P不具有相关性(P=0.068)。结论 HB-EGF在卵泡液中有表达,且与卵泡大小和卵泡液中E2水平、E2/P比值呈正相关性,与P水平无相关性。HB-EGF可能促进卵泡的生长发育,从而对卵母细胞的生长发育有一定的作用,通过检测卵泡液中HB-EGF可以对卵母细胞质量进行初步的评估。Objective To detect the expression level of heparin- binding epidermal growth factor( HB- EGF) in different sizes of ovarian follicles for in vitro fertilization- embryo transfer( IVF- ET) patients and to discuss the correlation and significance between HB- EGF and the estradiol( E2),progestogen( P) and E2/ P ratio in corresponding follicular fluid. Methods Clinical data of 58 cases in The First Affiliated Hospital of Harbin Medical University from April 2014 to September 2014 were selected,enzyme- linked immune sorbent assay( ELISA) is used for detection of HB- EGF expression in follicular fluid in patients who were undertaking the assisted reproduction because of fallopian tube factors or male causes while electrochemiluminescence( ECLIA) is used for detection of progestogen levels in this follicular fluid. A large ovarian follicle group( n = 32) and a small ovarian follicle group( n = 26) were designed and they were also as control groups to each other. Results ELISA test discovered that HB- EGF had expression in follicular fluid. The level of HB- EGF and E2 and E2/ P ratio in follicular fluid had positive correlations with the size of ovarian follicle. The level of HB- EGF and E and E2/ P ratio in the large ovarian follicle group was significantly higher than that of small ovarian follicle group(P〈0. 05),but P value was not relevant to the size of ovarian follicles( P = 0. 226). The level of HB- EGF and E2 and E2/ P ratio in the large ovarian follicle group had positive correlations with correlation coefficients of 0. 562 and 0. 487,namely the higher the HB- EGF value,the bigger the E2 and E2/ P will be,and they were not relevant to P value( P = 0. 335). In the small ovarian follicle group,the level of HB- EGF and E2 and E2/ P ratio had positive correlations with correlation coefficients of 0. 454 and 0. 728 that were not relevant to P value( P = 0. 068).
