摘　　要：临床药师通过参与颅内动脉瘤弹簧圈置入术后,对1例双联抗血小板药物治疗导致严重消化道出血患者,抗血栓治疗时抗血小板药物的选择及后续的药学监护分析,制订和实施个体化的药学监护,同时根据药物的临床疗效及时调整药学监护方案。临床药师以双联抗血小板药物和质子泵抑制剂的合理选用作为切入点,开展全程的药学监护,并及时发现、解决药物治疗时出现的问题,保证患者用药合理、安全、有效,提高临床治愈率。临床药师在临床药物治疗实践中积累支架置入术后应用抗血小板药物导致消化道出血患者的药学监护经验,为临床合理用药提供建议,并体现临床药师的价值。By antiplatelet drug selection in antithrombotic therapy and subsequent pharmaceutical care analysis in one case of severe alimentary tract hemorrhage caused by dual antiplatelet therapy after intracranial aneurysm spring ring implantation,clinical pharmacists formulated and implemented individualized pharmaceutical care to the patients and adjusted the pharmaceutical care scheme in time according to the clinical effects of drugs.The clinical pharmacists took reasonable selection of dual antiplatelet drugs and proton pump inhibitor as the entry point to carry out full-course pharmaceutical care,and to discover and solve problems during treatment to ensure the rationality,safety, and effectiveness of medication and improve the clinical cure rate.The clinical pharmacists also accumulated experience in the pharmaceutical care of alimentary tract hemorrhage patients caused by antiplatelet therapy after stent implantation,which would be helpful to provide advice to clinical rational drug use and to exemplify the values of clinical pharmacists.
