疏肝健脾方对非酒精性脂肪性肝炎患者肝纤维化指标的影响

摘　　要：目的:观察疏肝健脾方对肝郁脾虚型非酒精性脂肪性肝炎患者血清TGF-β1及肝纤四项的影响。方法:选择非酒精性脂肪性肝炎肝郁脾虚型患者80例,随机分为两组,其中疏肝健脾方治疗组40例,易善复对照组40例,疗程为24周。观察两组治疗后肝功能、血脂、超声学及血清TGF-β1、HA、LN、CIV、PIIINP。结果:两组治疗后肝功能、血脂较治疗前差异有统计学意义(P<0.01),组间治疗后比较差异有统计学意义(P<0.01)。治疗后疏肝健脾方组B超总改善率为90.00%,易善复组B超总改善率为77.50%,两组比较差异有统计学意义(P<0.05)。两组治疗后血清TGF-β1、HA、LN、CⅣ、PIIINP含量较治疗前差异有统计学意义(P<0.01),组间治疗后比较除LN外,差异有统计学意义(P<0.01,P<0.05)。结论:疏肝健脾方可抑制非酒精性脂肪性肝炎患者肝纤维化趋势,对疾病有一定保护作用。Objective： To observe the influence of Soothing Liver and Invigorating Spleen Decoction on transforming growth factor beta 1（ TGF- β1） and liver fibrosis markers of nonalcoholic steatohepatitis（ NASH） patients. Methods： 80 cases of NASH patients of depressed liver and deficient spleen type were randomly divided into two groups,including Soothing Liver and Invigorating Spleen Decoction treatment group（ 40 cases） and the essentiale control group（ 40 cases）. After 24 weeks,we investigated the changes of liver function,blood lipids,ultrasonics and serum TGF- β1,HA,LN,CIV and PIIINP. Results： There were significant differences in the scores of liver function,blood lipids in the two groups before and after treatment and between the two groups after treatment（ P〈 0. 01）. The total efficiency rate of the treatment group was 90. 00% and that of the control group was 77. 50%and there was significant difference（ P〈 0. 01） in ultrasonics. There were significant differences in the scores of TGF- β1,HA,CⅣ and PIIINP in the two groups before and after treatment and between the two groups after treatment（ P〈 0. 01,P〈 0. 05） except LN. Conclusion： Soothing Liver and Invigorating Spleen Decoction can inhibit liver fibrosis and protect the liver.
