颅脑外伤患者早期血清超敏C反应蛋白与伤后抑郁的相关性分析

摘　　要：目的探讨颅脑外伤患者早期血清超敏C反应蛋白(hs-CRP)与伤后抑郁的相关性分析。方法选取2012年4月至2015年4月我院收治的185例颅脑外伤患者为研究对象,检测所有患者入院时、入院12h、入院24h的血清hs-CRP水平,记录患者相关临床资料,按照伤后是否出现抑郁将患者分为抑郁组(n=48例)和非抑郁组(n=137例),比较两组患者相关临床资料的差异。结果 185例颅脑损伤患者中出现伤后抑郁的有48例(发病率25.9%);抑郁组中患者入院时格拉斯哥昏迷评分(GCS)低于非抑郁组(P<0.05)、低氧血症的比例高于非抑郁组(P<0.05);抑郁组中患者入院12h、24h的血清hs-CRP水平高于非抑郁组(P<0.05);回归分析提示入院12h的血清hs-CRP(OR=1.887,P<0.05)、和24h的血清hs-CRP(OR=1.452,P<0.05)是颅脑损伤患者中出现伤后抑郁的独立危险因素。结论血清hs-CRP与颅脑外伤后抑郁的发生具有一定的相关性,早期检测血清hs-CRP水平对于评估伤后抑郁的发生具有一定的临床意义。Objective To investigate the relationship between serum hypersensitive c-reactive protein（ hsCRP） and depression after brain injury. Methods 185 cases of brain injury patients in our hospital from April 2012 to April 2015 were choose as research subjects,serum hs-CRP levels on admission,12 h and 24 h of hospital admission were detected; relevant clinical data was also recorded; all patients were divided into depression group（ n =48 cases） and non-depression group（ n = 137 cases） according to depression after injury. The differences between the two groups of clinical information were compared. Results 48 cases showed depression after injury（ incidence25. 9%）; GCS score of depression group was lower than that of non-depression group（ P〈0. 05）,the proportion of hypoxemia in depression group was higher than non-depression group（ P〈0. 05）; serum hs-CRP levels of 12 h and24h of hospital admission in depression group were higher than those in non-depression group（ P〈0. 05）; Multiariable Logistic regression analysis showed that serum hs-CRP levels of 12h（ OR = 1. 887,P〈0. 05） and 24h（ OR =1. 452,P〈0. 05） of hospital admission were independent risk factors of depression after brain injury. Conclusion Serum hs-CRP has a certain relevance with depression after brain injury,early detection of serum hs-CRP levels has a certain clinical significance in evaluating depression after brain injury.
