丙戊酸钠治疗中型闭合性颅脑损伤后抑郁、焦虑的临床效果研究

摘　　要：目的分析丙戊酸钠治疗中型闭合性颅脑损伤后抑郁的临床效果。方法取86例中型闭合性颅脑损伤后抑郁患者为对象,根据就诊顺序随机分组,对症组治疗方式为常规治疗,丙戊酸钠组在常规治疗同时给予药物丙戊酸钠。对比两组治疗效果、汉密尔顿抑郁评分、神经功能缺损评分、安全性。结果丙戊酸钠组治疗效果明显比对症组好,且治疗后两组患者汉密尔顿抑郁评分、神经功能缺损评分均明显下降,但丙戊酸钠组下降更为显著,跟对症组治疗后对比差异具有显著性(P<0.05)。丙戊酸钠组虽有部分患者出现恶心呕吐,但较为轻微,不影响治疗,跟对症组相比差异不显著(P>0.05)。两组治疗前SAS、SDS两项评分差异不大(P>0.05);治疗后对症组SAS评分45.83±4.91、SDS评分44.92±5.22,丙戊酸钠组SAS评分34.80±5.23、SDS评分33.69±4.99,此两项评分与组间评分差异均较大(P<0.05)。结论丙戊酸钠治疗中型闭合性颅脑损伤后抑郁的临床效果显著,可有效改善抑郁症状和神经功能缺损症状,且用药不良反应轻微,可在临床推广应用。Objective To analyze the clinical effect of sodium valproate in the treatment of depression after moderate traumatic brain injury. Method 86 cases of moderate close brain injury accompanying with depression patients as the object,grouped according to the order of treatment,symptomatic treatment to conventional treatment,sodium valproate group in the conventional treatment while giving sodium valproate. The effect of treatment,Hamilton depression score,neurological deficit score and safety were compared between the two groups. Results The results of sodium valproate treatment group effect is obviously good than in the symptomatic group and after treatment in two groups patients in Hamilton depression score,neural function defect score were significantly decreased,but sodium valproate group decreased more significantly,with symptomatic group after treatment,the difference is significant（ P〈0. 05）. Sodium valproate group,although some patients had nausea and vomiting,are relatively minor and does not affect the treatment,and symptomatic group compared to the difference was not significant（ P〈0. 05）. Two groups＇ SAS and SDS scores before treatment were not difference（ P〉0. 05）. After treatment of symptomatic group of SAS score 45. 83＋4. 91,SDS score 44. 92 ＋ 5. 22,sodium valproate group,the scores of SAS 34. 80 ＋ 5. 23,SDS score 33. 69＋4. 99. Between the two scores of the experimental group scores were significantly different（ P 0. 05）.Conclusion sodium valproate in the treatment of moderate close brain injury accompanying with depression of significant clinical effect,can effectively improve depressive symptoms and symptoms of neurological deficit,and adverse drug reactions of the minor,can be popularized in clinical application.
