摘  要:目的:通过对肝硬化、轻微型肝性脑病(MHE)患者血清代谢物的分析,找出MHE特异性代谢物谱。方法:采用气相色谱-质谱技术(GC-TOFMS)和液相色谱-质谱技术(UPLC-QTOFMS)研究23例肝硬化失代偿期患者(肝硬化组)、29例MHE患者(MHE组)和50例健康人(对照组,NC)血清中的代谢物,应用主成分分析(PCA)和正交偏最小二乘法-判别分析(OPLS-DA)对代谢物谱数据进行模式识别分析。结果:用代谢组学方法鉴定了29个差异代谢物,其中21个代谢物上调,包括各种次级胆汁酸、胆酸盐、酪氨酸、山梨醇、神经酸、柠檬酸、肌酐等,以胆汁酸、胆酸盐增高最为显著。下调代谢物包括支链氨基酸、草酸、神经鞘氨醇、甲状腺素、维生素E、牛磺酸。结论:胆汁酸、胆酸盐的代谢紊乱影响脂质代谢,加重肝细胞毒作用;芳香族氨基酸/支链氨基酸比例失调、神经鞘氨醇含量下降是MHE患者发生认知功能改变的原因之一。Objective:To seek out the specific metabolite profiling of cirrhosis patients with minimal hepatic encephalopathy. Methods: serum metabolites of 23 cases of liver cirrhosis patients(cirrhosis group), 29 cases of patients with minimal hepatic en-cephalopathy(MHE group)and 50 healthy people(control group) were detected by using GC-TOFMS and UPLC-QTOFMS, the outcome raw data were analyzed by using principal component analysis(PCA) and orthogonal partial least squares- discriminant analysis(OPLS-DA) for pattern recognition analysis. Results: Twenty-nine different metabolites were identified, among them, 21 were increased, including a variety of secondary bile acids, cholate, tyrosine, sorbitol, nerve acid, citric acid, creatinine, etc., bile acids and bile salts increased the most; 8 were decreased, including branched-chain amino acids, oxalic acid, sphingosine, thyroxine, vitamin E and taurine. Conclusion: Metabolic disorders of the bile acids and bile salts affect the lipid metabolism and increase the hepatic cytotoxicity. Imbalance of aromatic amino acids and branched-chain amino acid ratio and sphingosine decreasing might be the reasons for the cognitive function disorders of MHE.
