术前焦虑抑郁对泌尿系癌症患者生活质量的影响

摘　　要：目的了解术前焦虑和抑郁对泌尿系癌症患者生活质量的影响。方法选取101例泌尿系癌症患者,在术前完成问卷调查表,其内容包括患者的人口学特征、临床特征、医院焦虑抑郁量表和癌症患者生活质量量表(EORTC QLQ-C30)。结果 101例中,28.7%的患者术前存在焦虑症状,16.8%的患者存在抑郁症状。焦虑组患者的躯体功能、情绪功能、认知功能、社会功能、总体健康状况得分低于非焦虑组患者,而疲倦、疼痛、食欲丧失、失眠和经济困难得分高于非焦虑组患者。抑郁组患者的躯体功能、情绪功能、认知功能、社会功能和总体健康状况得分低于非抑郁组患者。抑郁组患者的疲倦和失眠得分高于非抑郁组患者。结论术前焦虑和抑郁对泌尿系癌症患者的生活质量产生不利影响,应该根据焦虑和抑郁评分状况积极地进行围手术期的心理干预和社会支持。Objective The aim of this study was to evaluate the effect of preoperative anxiety and depression on quality of life among urinary system cancer patients. Methods 101 patients with urinary system cancer were selected.They participated in this survey by completing a detailed ques-tionnaire regarding their demographic data,clinical characteristics,Hospital Anxiety and Depression Scale questionnaire,and EORTC QLQ-C30 questionnaire. Results The prevalence of anxiety and depression was 28.7% and 1 6.8%,respectively.The scores of physical function,emotional function, cognitive function,social function and total health status in patients with anxiety were lower than those in patients without anxiety,while the scores of fatigue,pain,loss of appetite,insomnia,and financial difficulties were higher than those in patients without anxiet.The scores of physical func-tion,emotional function,cognitive function,social function and total health status in patients with depression were lower than those in patients without depression,while the scores of fatigue,pain, and insomnia were higher than those in patients without depressioon. Conclusions Preoperative anxiety and depression has negative effect on the quality of life of patients with urinary system canc-er.According to the results of preoperative anxiety and depression score,it is nessesary to offer pa-tients a positive psychological intervention and social support.
