高度近视患者眼底自发荧光的影像特征观察

摘　　要：目的研究高度近视患者眼底自发荧光的影像特征,探讨眼底自发荧光检查对高度近视患者眼底病变随访、诊疗的意义。方法回顾性系列病例研究。对2012年12月至2014年5月就诊于北京协和医院眼科门诊的高度近视患者27例(54只眼)采集视力、眼压、屈光度、眼轴、裂隙灯检查、眼底检查及眼底自发荧光检查结果,分析眼底自发荧光对高度近视患者眼底情况诊疗意义。结果 27例患者中15例(23只眼,占42.6%)存在不同程度的视盘旁弧形脉络膜萎缩灶及豹纹状眼底改变,其中视网膜脉络膜萎缩斑10例(13只眼,占24.1%),漆样裂纹9例(10只眼,占18.5%),脉络膜新生血管(CNV)5例(8只眼,占14.8%),黄斑裂孔2例(2只眼,占3.7%),视网膜出血3例(3只眼,占5.6%)。结论眼底自发荧光通过无创、便捷、实时地监测高度近视眼底表现,可为高度近视眼底病变的诊断、评估病情严重度及预后提供参考价值。Objective To observe and analyze the characters of fundus autofluorescence imaging in patients with high myopia. Methods This is a retrospective case series study. Twenty-seven high myopia patients were enrolled during their visits to Peking Union Medical College Hospital from December 2012 to May 2014. Among them were 23 females and4 males,ages ranging from 17 to 76 years（ mean 49. 44 ± 15. 78）. Visual acuity,intraocular pressure,refraction diopter,optic axis,slit lamp examination,funduscopic examination and fundus autofluorescence were measured or tested. Diopters ranged from- 6. 00 D to- 23. 75 D,with a mean diopter of- 13. 38 ± 4. 59 D. Optic axis ranged from 25. 46 mm to32. 73 mm with a mean axis length of 28. 73 ± 1. 65 mm. The use of fundus autofluorescence in follow up observation of high myopia were evaluated. Results Fifteen out of 27 high myopia patients（ 23 eyes,42. 6%） were found to have different levels of parapapillary atrophy and tessellated fundus. Among them,10 patients（ 13 eyes,24. 1%） showed retinal atrophy,9 patients showed（ 10 eyes,18. 5%） lacquer crack,5 patients（ 8 eyes,14. 8%） were found to have choroidal neovascularization,2 patients（ 2 eyes,3. 7%） were found to have macular hole,and 3 patients（ 3 eyes,5. 6%） were found to have retinal hemorrhage. Conclusion Fundus autofluorescence is a valuable non-invasive and real-time technique to follow up the fundus situation,and to assist early detection of minor lesions and guide treatment for high myopia patients.
