感染性眼内炎24例分析

摘　　要：目的探讨感染性眼内炎的病因、特点以及不同手术方式治疗眼内炎的临床疗效。方法对武汉大学人民医院眼科中心三病区2013年1月至2014年8月收治的24例(24只眼)感染性眼内炎患者的感染原因、病原学检查、治疗方法、治疗效果的进行回顾性分析。结果因眼外伤所致眼内炎12例,内源性眼内炎4例,感染性角膜炎2例,内眼手术后6例,其中白内障术后5例,玻璃体切除术后1例。病原学培养阳性例数3例。4例患者最终行眼球内容物剜除术,1例行角膜溃疡清创术及球内注药术,1例行前房注药术,1例行4次球内注药,18例患者行玻璃体切除术+球内注药术(其中1例最终行眼球内容物剜除术)。经治疗后有14例视力有不同程度提高。结论眼外伤是眼内炎的最常见原因,玻璃体切除+球内注药术能不同程度恢复患者视功能。Objective To analyze the etiology,characteristic,treatment and treatmenteffect of endophthalmitis.Methods This was a retrospective study,totally 24 cases endophthalmitis patients in Department of Ophthalmology,Renmin Hospital of Wuhan University from January,2013 to August,2014 underwent pathogenic examination,etiological examination and thetreatment and treatmenteffects were analyzed. Results The 4 major causes of infectious endophthalmitis of all 24 patients included ocular trauma（ 12 cases）,endogenous endophthalmitis（ 4 cases）,infectious keratitis（ 2 cases）,eye surgery（ 6 cases）,including 5 cases after cataract surgery and 1 case after vitrectomy. Pathogens were detected in3 cases,the total detection rate was 17. 6%（ 3 /17）. Fanally,4 patients underwent orbital exenteration,1 patient underwent corneal ulcer debridement combined with vitreous chamber infusion,1 patient underwent anterior chamber injection,1 patient underwent 4 times of itravitreal injection,18 patients underwent vitrectomy combined with vitreous chamber infusion. Visual acuity of 14 cases（ 51%） was improved visual acuity in varied degree. Conclusion The major reason of infectious endophthalmitis was ocular trauma,vitrectomy combined with vitreous chamber infusion can improve in varied degree.
