摘  要:目的采用微型营养评定法(MNA)联合蛋氨酸代谢相关指标评估老年2型糖尿病患者的营养状况。方法选取老年2型糖尿病患者124例为研究对象,应用MNA对其营养状况进行评价,同时检测常用营养指标(体重指数、白蛋白、前白蛋白)及蛋氨酸代谢指标[同型半胱氨酸、叶酸、维生素B12(VitB12)]。结果老年2型糖尿病患者营养不良、营养不良危险的发生率分别为14.5%、45.1%,而营养良好的患者占40.3%。Pearson相关分析结果显示,MNA与体重指数、白蛋白及前白蛋白显著相关。老年2型糖尿病营养不良组患者血同型半胱氨酸水平增高,同时叶酸、VitB12水平降低。结论老年2型糖尿病患者营养状况差,且随着年龄的增加,营养不良及营养不良危险发生率逐渐增高。适时检测老年糖尿病患者血浆同型半胱氨酸水平,联合MNA可全面评估老年2型糖尿病患者营养状况,为其合理的饮食治疗提供依据。Objective To evaluate the nutritional status in elderly patients with type 2 diabetes mellitus using mini nutritional assessment ( MNA ) and methionine metabolism markers. Methods 124 cases of elderly patients with type 2 diabetes were enrolled. The nutritional state of the patients was assessed by MNA. Common nutritional indexs ( BMI, albumin, prealbumin) and methionine metabolism markers ( homocysteine, folate, vitamin B12 ) were also detected. Results According to the scores of MNA, 14.5% of senile diabetic patients were malnutrition,45.1% were in the risk of malnutrition,and 40.3% kept a good nutritional state. Pearson correlation analysis showed that there was a significant correlation between prealbumin, BMI, albumin and MNA. The level of homocysteine increased in the malnutrition group,while the levels of folate and vitamin B12 decreased in the same group. Conclusion The preva- lence of malnutrition and the risk of malnutrition is higher in elderly patients with type 2 diabetes mellitus. In order to provide medical basis for its reasonable diet therapy,we can use MNA combined with methionine metabolism markers.
