摘 要:目的探讨氟化物预防正畸治疗中牙釉质脱矿的临床疗效及激光龋齿探测仪的临床应用价值。方法选择在新疆医科大学第五附属医院口腔科接受固定正畸矫治的患者63例,均采用直丝弓矫治技术,随机分为3组:含氟牙膏组采用含氟牙膏刷牙,于每日早、晚各1次,刷牙时间>3 min。氟保护漆组涂布氟保护漆,半年涂布1次。对照组采用日常口腔护理不做任何治疗。通过计算3组牙釉质脱矿指数和激光龋齿探测荧光值(DD值)评价正畸治疗前后牙釉质的脱矿程度。结果 6个月后含氟牙膏组、氟保护漆组临床视诊的牙釉质脱矿指数与对照组比较差异无统计学意义,而DD值低于对照组,差异有统计学意义(P<0.05)。12个月后,含氟牙膏组、氟保护漆组的牙釉质脱矿指数和DD值均低于对照组,差异均有统计学意义(P<0.05)。含氟牙膏组、氟保护漆组在6个月和12个月的牙釉质脱矿指数差异无统计学意义(P>0.05),氟保护漆组DD值低于含氟牙膏组,差异有统计学意义(P<0.05)。结论氟保护漆和含氟牙膏均能起到预防正畸治疗中牙釉质脱矿的作用,以氟保护漆作用更为明显,激光龋齿探测仪与探诊法相比所检测的牙釉质脱矿指数更可靠、更精准,在检测固定正畸治疗中牙釉质脱矿的效果更为显著。Objective To research the clinical effect of fluoride on enamel demineralization in orthodontic treatments.Methods A total 63 patients with fixed orthodontics which all belonged to the straight wire bow rectification technology were collected in the Fifth Affiliated Hospital of Xinjiang Medical University.Patients were randomized into three groups:patients in fluoride toothpaste group were used fluoride toothpaste more than three minutes twice daily.Patients in fluor protector group were used fluor protector once half a year.Other cases were in control groups adapting routine nursing without any treatment.Enamel demineralization index and DIAGNOdent were measured in three groups,and the degree of enamel demineralization before and after orthodontic treatment were evaluated in two groups.Results After treatment for 6 months,the enamel demineralization index is no significant difference among fluoride toothpaste group,fluor protector group and control group,while DD was lower than the control group(P0.05).After treatment for 6 months,the enamel demineralization index and DD in fluoride toothpaste group,fluor protector group were lower than the control group,and the differences were significant(P0.05).The enamel demineralization index was no remarkable difference between fluoride toothpaste group,fluor protector group at 6 and 12 months.But DD in fluor protector group was lower than fluoride toothpaste group on the degree oi demineralization.Conclusion Both fluor protector and fluoride toothpaste play a part in preventing the enamel demineralization in orthodontic treatment,and fluoride is more effective,DIAGNOdent can furnish more reliable and accurate measurement which is suitable for monitor of enamel demineralization in orthodontic treatment.
