摘 要:目的观察血凝酶联合氨甲环酸治疗高血压性基底核中等量出血的临床疗效。方法选取2010年7月—2014年2月湖北医药学院附属东风医院收治的高血压性基底核中等量出血患者104例,根据随机数字表法分为治疗组和对照组,各52例。两组患者入院后均给予基础治疗,对照组患者在此基础上给予氨甲环酸,治疗组患者在对照组基础上加用血凝酶;两组患者均持续治疗2个月。比较两组患者治疗前后神经功能缺损评分、格拉斯哥昏迷量表(GCS)评分、血液流变学指标,评估其治疗后患侧肢体功能,记录治疗期间不良反应发生情况。结果两组患者治疗前神经功能缺损评分、GCS评分比较,差异无统计学意义(P>0.05);治疗组患者治疗后神经功能缺损评分低于对照组,GCS评分高于对照组(P<0.05)。两组患者治疗前血浆黏度、高切还原黏度比较,差异无统计学意义(P>0.05);治疗组患者治疗后血浆黏度、高切还原黏度低于对照组(P<0.05)。治疗组患者治疗后患侧肢体功能分级优于对照组(u=8.113,P<0.05)。两组患者治疗期间均未出现明显不良反应。结论血凝酶联合氨甲环酸可有效改善高血压性基底核中等量出血患者神经功能、患侧肢体功能及血液流变学指标。Objective To observe the clinical effect of hemocoagulase combined with tranexamic acid on moderate hypertensive basal ganglia hemorrhage. Methods A total of 104 patients with moderate hypertensive basal ganglia hemorrhage were selected in Dongfeng Hospital Affiliated to Hubei College of Medicine from July 2010 to February 2014, and they were divided into control group and treatment group according to random number table,each of 52 cases. Based on basic treatment,patients of control group were given tranexamic acid,while patients of treatment group were given hemocoagulase combined with tranexamic acid; both groups treated for 2 months. Neurological function deficit score,GCS score,plasma viscosity and whole blood high shear reductive viscosity before and after treatment were compared between the two groups, disabled limb function grade of the two groups was evaluated, and incidence of adverse reactions during treatment was recorded. Results No statistically significant differences of neurological function deficit score or GCS score was found between the two groups( P 〉0. 05); after treatment, neurological function deficit score of treatment group was statistically significantly lower than that of control group,while GCS score of treatment group was statistically significantly higher than that of control group( P 〉0. 05). No statistically significant differences of plasma viscosity or whole blood high shear reductive viscosity was found between the two groups before treatment( P 〉0. 05),while plasma viscosity and whole blood high shear reductive viscosity of treatment group were statistically significantly lower than those of control group after treatment( P 〈0. 05). The disabled limb function grade of treatment group was statistically significantly better than that of control group( u = 8. 113,P 〈0. 05).
