弥漫大B细胞淋巴瘤患者MYC/BCL-2共表达情况及其临床意义分析

摘　　要：目的分析弥漫性大B细胞淋巴瘤(DLBCL)患者MYC/BCL-2共表达情况及其临床意义。方法选取2012年1月—2015年1月唐山市人民医院收治的首诊DLBCL患者30例,采用免疫组化法检测患者MYC和BCL-2表达情况,并分析不同临床特征DLBCL患者MYC/BCL-2共表达情况。结果 MYC阳性表达率为70.0%,BCL-2阳性表达率为53.3%,MYC/BCL-2共阳性表达率为30.0%。不同性别、临床分期、国际预后指数(IPI)评分及乳酸脱氢酶(LDH)水平DLBCL患者间MYC/BCL-2共阳性表达率比较,差异无统计学意义(P>0.05);结外受侵、非生发中心型(non-GCB)、未完全缓解DLBCL患者MYC/BCL-2共阳性表达率分别高于非结外受侵、生发中心型(GCB)、完全缓解患者(P<0.05)。结论 DLBCL患者MYC/BCL-2共阳性表达率较高,且常见于non-GCB、结外受侵、预后不良的DLBCL患者。Objective To investigate the coexpression and clinical significance of MYC/BCL-2 in patients with diffuse large B cell lymphoma. Methods A total of first- diagnosis patients with diffuse large B cell lymphoma were selected in the People＇ s Hospital of Tangshan from January 2012 to January 2015, immunohistochemical method was used to detect the expressions of MYC and BCL-2,and coexpression rate of MYC / BCL-2 was compared in patients with different clinical features.Results The positive expression rate of MYC was 70. 0%,of BCL-2 was 53. 3%,and the positive coexpression rate of MYC /BCL-2 was 30. 0%. No statistically significant different differences of positive coexpression rate of MYC / BCL-2 was found in patients with different gender,clinical staging,IPI scores or LDH levels（ P 〉0. 05）; positive coexpression rate of MYC / BCL-2of patients with lymphatic metastasis,non- germinal center B cell lymphoma and without complete remission was statistically significantly higher than that of patients without lymphatic metastasis,with germinal center B cell lymphoma and with complete remission,respectively（ P 〈0. 05）. Conclusion The positive coexpression rate of MYC / BCL-2 is relatively high in patients with diffuse large B cell lymphoma,and it is commonly expressed in patients with lymphatic metastasis,non- germinal center B cell lymphoma and without complete remission.
