性激素结合球蛋白与子宫内膜样腺癌发病关系的研究

摘　　要：目的检测性激素结合球蛋白(SHBG)在子宫内膜样腺癌、子宫内膜上皮内瘤变患者血清及组织中的表达,探讨SHBG与子宫内膜样腺癌发病之间的关系。方法选取子宫内膜样腺癌及子宫内膜上皮内瘤变患者各60患者和子宫内膜正常者60例,采用酶联免疫吸附(ELISA)法检测血清中SHBG水平,免疫组化SP方法检测内膜组织中SHBG的表达。试验组与对照组直接血清SHBG水平采用成组t检验,内膜组织中SHBG的表达采用卡方检验进行比较,P<0.05为差异有统计学意义。结果子宫内膜样腺癌、子宫内膜上皮内瘤变患者及健康对照血清水平分别为(36.41±20.26)ng/L、(59.28±23.69)ng/L及(82.84±19.51)ng/L,两组与对照组进行比较差异均有统计学意义(t值分别为4.775、2.364,均P<0.05)。三组内膜组织中SHBG表达阳性率分别为18.33%、43.33%及61.67%,两试验组与对照组比较差异均有统计学意义(χ2值分别为23.47、4.04,均P<0.05)。随着病变加重血清及组织中SHBG逐渐下降。结论血清及组织中SHBG表达的异常与子宫内膜样腺癌的发生可能有密切的关系,SHBG可能成为子宫内膜样腺癌早期诊断的新型标记物。Objective To examine the expression of sex-hormone binding globulin (SHBG) in serum and tissues of endometrial adenocarcinoma, atypical endometrial hyperplasia and normal endometrium, and discuss the relationship between the expression of sex-hormone binding globulin and morbidity of endometrial adenocarcinoma. Methods We chose 60 cases of endometrioid adenocarcinoma patients, 60 cases of atypical endometrial hyperplasia, and 60 cases of normal endometrium as a control group. The levels of SHBG in serum was determined by enzyme linked immunosorbent assay, the expression of SHBG in tissues was detected using immunohistochemical method. The serum SHBG level of the experimental group and the control group were tested by t, and the expression of SHBG was compared with the chi square test. P<0.05 was statistically significant. Results The serum level of SHBG increased in these three groups, they were (36.41±20.26) ng/L, (59.28±23.69) ng/L and (82.84±19.51) ng/L, the t values of the two groups were as follows, 4.775, 2.364, P<0.05, and the positive expression rate of SHBG in tissues of three groups cases also had increasing trend, the positive expression rates were 18.33%, 43.33%, 61.67%, respectively. Comparison of two groups of experimental group and control, Chi square values were 23.47 and 4.04, P<0.05. Conclusions The descend of SHBG in serum and tissues might have a close relationship with the morbidity of endometrial adenocarcinoma. SHBG might be a new marker for early diagnosis of endometrial carcinoma.
