摘　　要：目的评价冷圈套治疗结直肠小息肉的疗效及安全性。方法检索Medline数据库、PubMed数据库、Cnki数据库、万方数据库，查询有关冷圈套治疗结直肠小息肉的随机对照临床试验文献，检索时限为建库开始至2014年12月，由2名评价者独立选择试验、提取资料和评估方法学质量，然后采用RevMan5．2版软件进行Meta分析。结果共有5篇随机对照临床试验文献、总计1712枚结直肠小息肉（直径2—9mm）纳入分析范围。Meta分析结果显示：冷圈套组的息肉完整切除率与传统方法组差异无统计学意义（RR=1．01，95％CI：0．99—1．03，P=0．35）；冷圈套组的息肉切除时间显著低于传统方法组（SMD=-1．80，95％CI：-2．60--0．99，P〈0．0001）；冷圈套组的不良反应发生率显著少于传统方法组（RR=0．24，95％CI：0．10～0．55，P=0．0007）。结论冷圈套切除结直肠小息肉与传统切除方法具有相同的临床疗效，但更安全，且用时更短，具有较高的临床应用价值。Objective To evaluate the efficacy and safety of cold snare for small coforectal polyps. Methods Databases of Medline, Puhmed, Cnki, and Wanfang were searched for randomized controlled clinical trials on cold snare polypectomy for small colorectal polyps from the start time of database to December 2014. Studies were extracted and the quality of methodology was assessed by two reviewers independently. RevMan 5.2 software was used to complete meta-analysis. Results A total of 5 clinical randomized controlled trials including 1 712 colorectal small polyps （diameter 2-9 mm） were analysed. Meta analysis showed that there was no significant difference in complete resection rate between cold snare group and the traditional method group [ RR = 1.01,95% CI：0. 99-1.03, P = 0. 35 ]. The mean resection time for poly removal was significantly shorter in cold snare polypectomy compared with conventional polypectomy [ SMD = - 1.80,95% CI： - 2.60- - 0. 99, P 〈 0. 000 1 ]. Complication rates were significantly lower in cold snare group than in the traditional method group [ RR = 0.24,95% CI： 0. 10-0. 55, P =0. 000 7 ]. Conclusion Compared with the traditional methods, cold snare resection of colorectal small polyps has the same clinical efficacy with more safety,less operation time and higher value in clinical application.
