摘  要:目的:观察上颌后牙缺失后牙槽骨高度不足实施上颌窦内提升植骨同期植入种植体的临床效果。方法:对28例(男19人,女9人),年龄平均56.8岁的上颌后牙牙列缺损患者,术前CBCT显示牙槽骨高度平均为4.2mm (3mm-5.6mm),局麻下进行上颌窦内提升手术和植骨,并同期植入ANKYLOS系列种植体21枚和奥齿泰TS-Ⅲ系列种植体13枚(共计34枚种植体)。术后随访12个月~38个月,平均23个月,观察方法为临床检查和X线检查,评价种植体周骨密度和种植体颈部骨高度存留情况。结果:28例上颌窦底提升高度平均为5.8mm (8.6mm-4.3mm),共植入了34枚种植体,术后6个月均已完成种植修复,术后无明显的并发症和任何不良反应,所有种植体稳定无松动、脱落,行驶咀嚼功能正常,无上颌窦炎症发生,牙龈乳头无红肿,X线检查显示种植体周牙槽骨高度正常,无明显吸收,种植体周围无阴影,上颌窦粘膜均匀被抬起,骨替代材料均匀分布种植体根部周围。结论:上颌窦内提升术并植骨同期植入种植体,手术创伤小,节省时间,临床操作相对简单易行,后牙牙列缺损种植修复后咀嚼功能明显提高,临床效果良好可靠。Objective: To observe the clinical effect of maxillary sinus floor elevation with simultaneous placement of dental implants for repairing maxillary posterior teeth loss and maxilla bone insufficiency. Methods: 28 cases(19 males, 9 females), whose average age is 56.8 years old, suffered from maxillary posterior teeth loss with the average residual bone height 4.2mm(3mm-5.6mm) by CBCT before opera-tion. Maxillary sinus floor elevation surgery and bone grafting surgery were performed under local anes-thesia, in the same time 21 ANKYLOS implants and 13 OSTEM TS-Ⅲ implants (34 implants in total) were implanted. Patients were followed up for 12 to 38 months (23 months on average) after surgery. The effects were evaluated the peri-implant bone density and the remaining state of the bone height of crestal-implant by clinical examination and X-ray examination. Result: 28 cases of posterior teeth and maxilla bone insufficiency were lifted maxillary sinus floor height to 5.8mm (average), varing from 8.6mm to 4.3mm, with bone grafting simultaneous placement of 34 dental implants. The implant restoration were completed within 6 months. There were no significant complications and any adverse reactions. All the implants were stable without loosening or falling-off, with normal chewing function, no sinus inflammation, no swelling gingival papilla, X-ray examination showed normal peri-implant alveolar bone height, without no significant bone absorption, no shadow around the implant, and sinus floor mucosa was evenly lifted, bone substitute material was uniformly distributed around the implant root. Conclusion: Applying maxillary sinus floor elevation with simultaneous placement of dental im-plants for repairing maxillary posterior teeth loss and maxilla bone insufficiency can reduce surgical trauma, save time, and its clinical operation is relatively simple, and the chewing function could be im-proved significantly.
