抑郁对轻中度癌痛患者回顾性记忆功能的影响

摘　　要：目的探讨轻中度癌痛合并抑郁状态患者的回顾性记忆功能,试图了解抑郁对轻中度癌痛患者回顾性记忆的影响。方法分别以30例轻中度癌痛合并抑郁状态患者和30例轻中度癌痛不合并抑郁状态患者为研究对象,对其进行MMSE,DS,VFT等神经心理学背景测试及中文听觉词学习(Mini Mental State Examination,AVLT)任务为回顾性记忆测查。并对30例轻中度癌痛合并抑郁状态患者进行抗抑郁治疗一月后进行HAMD、MMSE、AVLT测查。结果与轻中度癌痛不合并抑郁状态患者相比,轻中度癌痛合并抑郁状态患者在AVLT(t1=9.227,t2=8.968,P<0.01)方面差异有统计学意义。轻中度癌痛合并抑郁状态患者经抗抑郁治疗后,AVLT水平较前好转(t^3=-2.16,P<0.05,t!4=-5.16,P<0.01)。抑郁程度与MMSE(r=-0.77,P=0.00),R-AVLT(r=-0.83,P=0.00),D-AVLT(r=-0.79,P=0.00)成绩负相关。结论轻中度癌痛合并抑郁状态患者与轻中度癌痛不合并抑郁状态患者相比存在回顾性记忆障碍,且经抗抑郁治疗后其记忆水平较前提高,提示抑郁状态可能与癌痛患者的回顾性记忆障碍的发生有关。Objective To investigate retrospective memory in advanced cancer pain patients with mild to moderate pain and depression,and to find out how depression influence retrospective memory. Methods Thirty mild to moderate cancer pain patients with depression and thirty patients with same baseline characteristics but without depression were administered with a neuropsychological battery of tests including MMSE,DS,VFT,AVLT. The thirty cancer pain patients with depression took anti-depression therapy for one month,then had with depression. After anti-depression therapy,the AVLT score of cancer pain patients with depression improved more than before（ t^3=- 2. 16,P〈0. 05,t^4=- 5. 16,P〈0. 01）. The level of depression had negative correlation with cognition and memory,MMSE（ r =- 0. 77,P = 0. 00）,R-AVLT（ r =- 0. 83,P = 0. 00）,D-AVLT（ r =- 0. 79,P = 0. 00）. Conclusions Cancer pain patients with depression show disorder of retrospective memory,and the level of retrospective memory improves after anti-depression therapy,which means depression may lead to disorder of retrospective memory in cancer pain patients.
