摘  要:目的探讨分子探针转铁蛋白超顺磁性氧化铁颗粒(Tf-SPION)在肝癌早期介入治疗中的应用价值。方法2009年8月至2013年12月临床确诊的肝癌患者120例,均给予早期经导管动脉化疗栓塞介入治疗,在介入治疗前后均进行基于分子探针Tf-SPION的MR诊断。观察病灶部位的MR影像学特征,特别是对肿瘤富血供组织与肿瘤乏血供组织进行区别判断。在介入治疗后1个月与3个月进行肿瘤治疗效果的判断。结果所有患者均顺利完成MR检查,肝癌病灶部位在T1WI上全部为低信号,T2WI上病灶表现为高信号,周围肝组织的平均表现弥散系数(ADC值)(1.73±0.46)×10^(-3)mm^2/s,肝癌病灶部位的ADC值(1.54±0.40)×10^(-3)mm^2/s。肝癌病灶部位的ADC值明显低于周围肝组织,差异有统计学意义(P<0.05)。肿瘤富血供组织的ADC值明显低于乏血供组织,差异有统计学意义(P<0.05)。介入治疗3个月后的有效率为87.5%,明显高于介入治疗1个月的有效率75.0%,差异有统计学意义(P<0.05)。介入治疗1个月后患者的ADC值为(1.63±0.64)×10^(-3)mm^2/s,介入治疗3个月后的ADC值为(1.78±0.31)×10^(-3)mm^2/s,均明显高于介入治疗前的ADC值(P<0.05)。结论肝癌早期介入治疗能取得比较好的预后效果,分子探针Tf-SPION在介入中的应用能指导治疗方案的选择与疗效的评定,值得推广应用。Objective To investigate the applying values of Molecular Probes Tf- SPION for the early intervention in the treatment of liver cancer. Methods Selected clinical diagnosed 120 liver cancer patients from August 2009 to December 2013,there were all given transcatheter arterial chemoembolization early intervention. There were all given the molecular probes Tf- SPION of MR diagnostic applications before and after treatment the intervention. Results All patients received successfully MR examination. The liver lesion site on T1 WI were all low signal,and high signal lesions on T2 WI. The ADC value of liver lesion site were( 1. 54 ± 0. 40) ×10-3mm2/s,and the ADC value of the surrounding liver tissue were( 1. 73 ± 0. 46) ×10-3mm2/s,which had significant differences( P〈0. 05). The ADC value in the hyper- vascular tumor tissue were significantly lower than the less vascular tumor tissue with significant differences( P〈0. 05). After interventional treatment of 1 month,the effective rate was 75. 0 %. After interventional treatment of 3 months,the effective rate was 87. 5 %,compared to significant differences( P〈0. 05). The ADC value in the Interventional treatment of 1 and 3 month were( 1. 63 ± 0. 64) ×10-3mm2/s and( 1. 78 ±0. 31) ×10-3mm2/s,which were significantly higher than the before intervention of ADC values( P〈0. 05). Conclusion Early interventional therapy can achieve relatively better prognosis. Molecular Probes Tf- SPION for the early intervention can guide treatment programs and evaluation efficacy that should be widely applied.
