改良平面导板在青少年安氏Ⅱ类1分类错矫治中的应用

摘  要:目的:探讨固定矫治器配合改良平面导板矫治青少年安氏Ⅱ类1分类错的临床疗效。方法:选择低角或均角的安氏Ⅱ类1分类错牙合畸形患者12例作为实验组,男6例,女6例,年龄12-17岁,平均14.5岁。采用直丝弓固定矫治,治疗中辅助使用改良上颌平导。选择与实验组相匹配的12例患者作为对照组,仅使用弓丝打开咬合。咬合打开后,制取模型,并进行模型测量分析,分析咬合打开时间及咬合打开量。治疗结束后进行头影测量,评价治疗效果。结果:模型测量分析显示:改良平面导板打开咬合效果显著,覆牙合平均减小2.8 mm,覆牙合平均打开时间为3.75个月,短于对照组,差异有统计学意义(P<0.05)。治疗前后头影测量结果显示:U1-SN由112o减小到101.8o(P<0.05),而U1-L1由109.6o增加到125.3o(P<0.05)。结论:在青少年安氏Ⅱ类1分类错矫治中辅助使用改良平面导板,能快速打开咬合,缩短疗程;同时,能提供较稳定的支抗,达到满意的治疗效果。Objective:To evaluate the curative effect of modified bite plate in the treatment of youth Angle class Ⅱ Division 1malocclusion.Methods:12 Angle class Ⅱ Division 1 patients with low or average mandibular plane angle were collected(6 male,6 female,12-17 years old,mean 14.5 plate.Another 12 matched patients were collected as control group to receive straight wire appliances only.The overbite change and the time needed for bite opening was measured and caculated by study models after the deep bite corrected.Results:Models analysis showed that the mean overbite reduction in the experiment group was 2.8mm,the overbite decreased significantly.The average time needed for bite opening in experiment group was 3.75 months,and was significantly shorter than control group.Pre and post-treatment cephalographs showed that the U1-SN angle reduced from 112° to 101.8°(P〈0.05) and the U1-L1 angle changed from 109.6° to 125.3°(P〈0.05).Conclusion:Modified bite plate was effective on bite opening and anchorage control in the treatment of youth Angle class Ⅱ Division 1 malocclusion.
