摘 要:目的:观察蛛网膜下腔出血(SAH)患者早期血清中多种生长因子(GFs)表达水平的变化,探讨其对SAH患者预后判断的临床意义。方法:选取39例健康人作为照组,选取41例SAH患者作为治疗组。治疗组SAH患者根据预后情况,分为预后良好组(A组)与预后不良组(B组)。全部SAH患者于发病后8h,24h,72h分别采集血样,应用ELISA法检测生长因子(GFs)中血管生成素1(Ang-1),Ang-2,Ang-1/-2,Tie-2,血管内皮生长因子(VEGF)的含量。结果:与正常对照组比较,SAH患者血清GFs水平随时间发病时间延长均逐渐增高。全部41例SAH患者中,预后良好组(A组)含患者26人,预后不良组(B组)含患者15人。预后不良组与预后良好组比较,出血后8h和24h血清中Ang-1水平显著增高(P<0.01),VEGF于出血后24h和72h水平显著增高(P<0.05);Ang-2水平于出血后72h明显升高(P<0.05);Ang-1/-2,VEGI和Tie-2无统计学差异。结论:SAH患者早期血清中生长因子中Ang-1,Ang-2和VEGF明显改变,在预后不良患组者中尤其明显。检测早期SAH患者血清中生长因子中Ang-1,Ang-2和VEGF水平,有助于对患者预后的判断。Objective: To observe the change of serum levels of growth factors(GFs) in subarachnoid hemorrhage(SAH) in the early time, to discuss the clinical significance of these changes for prognosis judgment. Methods: Selection of 39 cases of healthy people as a control group, 41 patients with SAH were selected as the treatment group. Treatment group according to the prognosis of patients with SAH were divided into good outcome group(group A) and poor outcome group(group B). Blood samples of patients with SAH were collected at 8 h, 24 h and 72 h. The serum Ang-1 and Ang-2, Ang-1/-2, Tie-2, VEGF, VEGI were measured by commercially available enzyme-linked immunosorbent assay. Results: Compared with the control group, serum GFs levels in patients with SAH increased gradually with time passing. In all of the 41 patients with SAH, the good outcome group(group A) included 26 cases, and the poor outcome group(group B) included 15 cases. Patients with poor outcome showed a significant rise of Ang-1 levels at 8h and 24 h post SAH(P〈0.01)and increased of VEGF at 24 h and 72 h post SAH P〈0.05)compared to those with good outcome. Ang-2 was found significantly higher at 72h(P〈0.05). The Ang-1/-2, VEGI and Tie-2 were found no statistically significant. Conclusion: Serum Ang-1, Ang-2and VEGF levels were significantly altered in patients with SAH in early time, and especially in those with poor outcome. The serum Ang-1, Ang-2 and VEGF detection may be useful for the prognosis evaluation in patients with SAH in early time.
