牙周脓肿不同方法治疗的临床疗效研究

摘　　要：目的对不同方法治疗牙周脓肿的疗效进行比较,并分析其临床疗效。方法选取2009年2月-2013年2月牙周脓肿患者160例,根据治疗方法的不同,将其分为4组,每组各40例,其中A组切开引流后全身使用甲硝唑或替硝唑静脉滴注治疗,B组使用α糜蛋白酶治疗,C组使用盐酸米诺环素软膏治疗,D组使用布洛芬缓释凝胶治疗,治疗前检测4组患者龈沟出血指数(SBI)、牙周袋深度(PD),并比较4种方法的临床疗效差异与4种方式治疗后的复发程度;采用SPSS13.0软件对数据进行统计分析。结果 4组治疗前的龈沟出血指数(SBI)、牙周袋深度(PD)基线一致,治疗后各组内与治疗前有明显改善(P<0.05);A组治疗有效率为75.0%、B组为95.0%、C组为82.5%、D组为97.50%,B、D明显优于A、C组(P<0.05),A组复发率为40.0%、B组为7.5%、C组为30.0%、D组为5.0%,A、C组复发率高于B、D组(P<0.05)。结论α糜蛋白酶和布洛芬缓释凝胶治疗牙周脓肿效果显著。OBJECTIVE To observe the clinical efficacies of different methods in treatment of periodontal abscess.METHODS A total of 160 patients with periodontal abscess who were treated in the hospital from Feb 2009 to Feb2013were enrolled in the study and divided into four groups according to the treatment methods,with 40 cases in each.The group A was treated with systemic intravenous drip of metronidazole or tinidazole after the incision and drainage,the group B was treated withα-chymotrypsin,the group C was treated with minocycline hydrochloride ointment,and the group D was treated with ibuprofen sustained release gel.The sulcus bleeding index（SBI）and periodontal pocket depth（PD）of the four groups of patients were detected before the treatment,and the clinical efficacies and recurrence rates were compared among the four methods after the treatment;the statistical analysis of data was performed with the use of SPSS13.0software.RESULTS The SBI and PD of the four groups were the same before the treatment,which were significantly improved in each group after the treatment（P〈0.05）.The effective rate of treatment was 75.0%in the group A,95.0%in the group B,82.5%in the group C,97.50%in the group D;the effective rate of treatment was significantly higher in the group B and D than in the group A and C（P〈0.05）.The recurrence rate was 40.0%in the group A,7.5%in the group B,30.0%in the group C,5.0%in the group D;the recurrence rate was higher in the group A and C than in the group B and D（P〈0.05）.CONCLUSIONα-chymotrypsin and ibuprofen sustained release gel can achieve significant clinical effects on treatment of the periodontal abscess.
