摘  要:目的探讨角巩膜物理特性及眼前段结构参数对角膜生物力学的影响和作用。方法横断面研究。对2012年5-9月在河南省立眼科医院用眼反应分析仪(ORA)对拟行激光屈光近视矫正的人群共187例(187只眼)进行角膜生物力学测量,获取角膜滞后量(CH)和角膜阻力因子(CRF)生物力学指标。使用Oculyzer眼前节分析系统测量中央角膜前弹力层密度(CCBD)、中央角膜基质密度(CCSD)、房角宽度(ACA)、前房宽度(ACW)和前房深度(ACD)等参数指标。采用Schoitz压陷眼压计测量眼压后,再根据转化量表换算出巩膜硬度系数E。将CH和CRF分别与上述指标进行相关分析和多重线性回归分析,探讨其相关性及对角膜生物力学影响的差异。结果CH和CRF与CCBD呈负相关性,差异有统计学意义(P〈0.01),与巩膜硬度系数E呈正相关性,差异有统计学意义(P〈0.05),而与CCSD和眼前段结构参数的线性相关分析差异无统计学意义(P〉0.05)。多重线性回归分析发现,CCBD和巩膜硬度系数E是显著性因子(P〈0.05),且CCBD对角膜生物力学性能的影响较大。结论中央角膜基质密度和巩膜硬度对角膜力学性能的影响较大,而眼前段结构与其无明显相关,角膜生物力学可能更多地与其本身和巩膜的特性有关,从而为进一步认识角巩膜手术和疾病对角膜生物力学的影响提供了依据和基础。Objective To investigate the effects of comeoscleral physical character and structure of ocular anterior segment on comeal biomechanical property. Methods A cross-sectional study. One hundred and eighty-seven subjects (187 eyes) who were scheduled for myopia refractive surgeries were performed by ocular response analyzer (ORA) and oculyzer anterior segment system. Comeal hysteresis (CH), corneal resistance factor (CRF), central corneal Bowman's membrane density (CCBD), central cornea stroma density (CCSD), anterior chamber angle (ACA), anterior chamber width (ACW) and anterior chamber depth (ACD) were measured. Scleral rigidity coefficient was evaluated by Schoitz tononometer. The correlation analysis was applied between CH, CRF and above parameters. Results CH and CRF correlated negatively with CCSD (P 〈0.01), but correlated positively with scleral rigidity coefficient (P 〈0.05). There had no statistically correlations between CH, CRF and other parameters (P 〉0.05). A multiple linear regression revealed CCSD and scleral rigidity coefficient were significant factors (P 〈0.05) and CCSD contributed more to CH and CRF. Conclusions Corneal biomechanical property correlated with CCSD and scleral rigidity, but has no significant correlations with CCBD and ocular anterior segment parameters. It can be interfered that corneal biomechanical property maybe more associated with feature of cornea and sclera, thus providing further acquaintance of corneal biomechanism due to comeoscleral diseases and surgeries.
