摘  要:目的:通过3D-TOF-MRA调查人群中大脑前动脉A1段发育不良的发生率以及其与前交通动脉瘤发生率的相关性。方法:2009年1月至2012年12月8013例患者行3.0T 3D-TOF-MRA检查并纳入研究,以容积重建(VR)及轴位原始图像作为诊断标准,对大脑前动脉A1段发育不良和前交通动脉瘤的发生率及发生部位进行统计。结果:共发现138例前交通动脉瘤患者;425例被诊断为大脑前动脉A1段发育不良,其中60例伴发前交通动脉瘤。在有大脑前动脉A1段发育不良的患者中前交通动脉瘤的发生率高达14.1%,明显高于无大脑前动脉A1段发育不良的患者中的前交通动脉瘤发生率(1.0%)(P<0.001)。结论:3D-TOF-MRA可以清晰地显示大脑前动脉A1段发育不良与前交通动脉瘤的形态细节,大脑前动脉A1段发育不良伴随着较高的前交通动脉瘤形成可能。Purpose: The aim of this study was to detect and assess A1 segment hypoplasia based on a large case series and investigate the potential links with AcomA aneurysm occurrence using magnetic resonance angiography (MRA). Methods: Between January 2009 and December 2012, 8013 patients underwent 3D-TOF-MRA examination. MRA was performed using a three-dimensional time-of-flight technique (3D-TOF) with volume rendering (VR) reconstruction. The presence and location of A1 segment hypoplasia and AcomA aneurysm was reviewed based on VR images and source images. Results: Among the 8013 patients examined and reviewed, 138 patients were identified with AcomA aneurysms, 425 patients were defined with A1 segment hypoplasia, among whom 303 were with right-sided A1 hypoplasia and 122 with left-sided A1 hypoplasia. Sixty of these 425 patients were confirmed with AcomA aneurysms, among them 49 were with right-sided A1 hypoplasia and 11 with left-sided A1 hypoplasia. The prevalence ofAcomA aneurysm with A1 segment hypoplasia was 14.1%, which was much higher than that (1.0%) of AcomA aneurysm without A1 segment hypoplasia (P〈0.001). Conclusion: The anatomical relationship between A1 segment hypoplasia and AcomA aneurysms was visualized by MRA, which displayed anomalies with sufficient clarity to enable diagnosis. The incidence of right-sided A1 segment hypoplasia accompanied with AcomA aneurysm was much higher than that of left-sided. Intracranial AcomA aneurysm development appeared to be associated with A1 segment hypoplasia.
