摘　　要：目的:研制冠心病合并常见慢性病的中医证候标准化测量工具。方法:遵循"据象辨证、病证结合"原则,应用文献学、量表学方法,构建问卷内容与结构,通过德尔菲法筛选修订条目,进行预调查和信效度评价。结果:形成以病程发展为横断面,包含发病因素、主症、兼症、实验室检查、舌脉象等维度的5个合并病证候调查问卷,信度效度良好。结论:基于中医意象思维设计的临床专家调查问卷可为冠心病合并常见病的证候学研究提供标准化工具。Objective：To design TCM syndrome standardized measurement tools for coronary artery heart disease（CHD）companied with common chronic diseases.Methods：Followed the principles of ＇syndrome differentiation according to imagery,and combination of disease and syndrome＇,the methods of philology and scale were used to design the content and structure of the questionnaire.Then,the Delphi method was used to filter and revise the options,so as to implement pre-survey and evaluate its reliability and validity.Results：Using disease progress as the cross section,syndrome questionnaires of five common chronic diseases were designed containing various dimensions,such as pathogenesis,primary symptom,accompanied symptom,laboratory test,tongue and pulse condition,etc.,which all showed good reliability and validity.Conclusion：Designed upon TCM imaginary thinking mode,this questionnaire for clinical specialist could provide standardized tools for syndrome study of CHD companied with common chronic diseases.
